**About the Role**
Reporting to the program manager, the Health & Wellness Assistant, will be responsible for supporting vulnerable clients in maintaining their overall health, wellness, and tenancy through auxiliary case management. This may include supporting treatment plans, training sessions on overall wellbeing, and referring to service delivery providers as necessary.
Their main goal is to ensure that clients are well equipped to achieve stability in the recovery of their mental health challenges. This opportunity grant student experience within a service care model for individuals experiencing onset of psychosis. The student will connect with different community mental agencies and support transitions in care between the EPI program and various healthcare agencies in the community.
**Duties and Responsibilities**:
- Support clinical assessments and person-centered treatment plans which may have a focus on geriatric care and/or trauma
- Support time limited, short term case management with the appropriate mental health community services
- Support psychiatric consultation and connect with families, physicians, pharmacists and other service providers as necessary
- Complete at least one project that involves the development of teaching tools used for client activities related to mental health promotion
- Incorporate crisis management skills in programming materials
- Provide comprehensive, holistic, person and family-centered access to services
- Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
- Understand and be able to integrate a philosophy of client-centered care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Maintain professional documentation in client files kept in SHIP’s record management system
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
**Qualifications**:
- Current Enrollment in Post-secondary education in Social Work, Social Services, Health Services, or equivalent
- Any experience providing assessments on individuals with mental health challenges is an asset
- Any knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis is an asset
- Any previous placement experience is an asset
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
**Reporting Function**:
The Health & Wellness Assistant will report directly to the Program Manager. They will work with both internal and external partners and will interact with all levels within the organization.
They will be a support system, resource, and an advocator for the team and Company.
**Working Conditions and Physical Requirements**:
- Hours are as arranged by the program Manager, typically held on Monday - Friday, 8:30am - 4:30pm but may vary depending on client needs
- Average amount of physical effort is required.
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.
