The Opportunity:
The Practice Lead is a position responsible for providing onsite operational support related to operational practices and standards to enhance the person-centered care provided to residents and their families across AgeCare. The Practice Lead plays a key role in the development and implementation of practice initiatives designed to support quality improvement and evidence-informed care for residents across the continuing care spectrum.

This position is accountable for supporting key performance indicators as identified through AgeCare’s Strategic Objectives, and through the organizations Performance Dashboard and Balance Scorecards. This includes Finance, Resource Management, Standards Compliance and Reporting, Collaborative Relationships/Partnerships and Quality Improvement initiatives to enhance the quality of life and experience for residents with the support of their families.

This position will lead change and support evidence-informed practice by developing/maintaining strong working relationships with key stakeholders and leaders across AgeCare and other continuing care partners including but not limited to Resident/ Family, Executive Directors’, Directors of Care, Managers and frontline teams/staff, physicians, pharmacists, and allied health professionals. Contributes to planning, design, coordination, implementation, and evaluation of programs, required operational practices (ROPs), priority initiatives, and quality improvement activities such as standardization and sustainment of organizational programs and enhancing appropriateness of their use.

Key Responsibilities:

- Ensures all AgeCare Continuing Care communities are providing exemplary person-centered care through skilled, knowledgeable, responsible care teams throughout all of AgeCare’s care models (SL3, SL4, SL4-D, LTC, Memory Care/Dementia, Respite and Private Assisted Living).
- Expert knowledge of the Continuing Care Health Services Standards within Alberta and British Columbia
- Supports implementation of transformational initiatives that support the organization’s goals in order to enhance quality clinical outcomes, improve resident safety, establish organization-wide leading practices, and improve the cost-effectiveness of care.
- In collaboration with the Director, Care Practices and Standards supports and ensures effective Incident Reports Management, system safety analysis and resulting recommendations are appropriately actioned and communicated.
- Provides clinical direction, supervision and coordination, as appropriate, for clinical leadership, front-line care staff, and consultants.
- Identifies, evaluates and monitors all service continuity risks related to the community. This includes, all care, and hospitality services inclusive of environmental and facility risks.

Qualifications:
Education and Experience:

- Bachelor of Nursing, with current CRNA or BCCMN registration
- Must be fluent in English, both orally and in writing
- Minimum of five (5) years advanced clinical care experience in a complex health care

environment, preferably with an exposure to the continuing care sector
- Experience in Infection, Prevention & Control an asset
- Demonstration of progressive leadership roles

- A sound understanding of all relevant Legislation, Regulations, Standards and Acts, e.g.,

Regional Service Delivery Standards, Accommodation Standards, Nursing Home Act, FOIP,

PIPEDA, Ethics, HIA, HPA, Professional Standards of Practice, and PPCA
