**_ Program Overview - Supportive Housing, Community Care_**
- The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues. The program provides onsite psychosocial, primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support._

**_What You Will Do_**
- _
Access client needs and develop and implement a plan of care or services with short and long-term objectives, with ongoing monitoring to ensure clients keep their housing._
- _ Provide short-term intensive crisis case management services which includes assessment, supportive counselling, referrals to community resources._
- _ Support the Supervisor and Manager with implementing the operational responsibilities of programs, services, and initiatives associated with Cedarvale program._
- _ Provide daily oversight of program objectives and supports with training of staff & student placement._
- _ Work directly with the program participants to ensure they are meeting program expectations._
- _ Provide support to staff regarding program initiatives, team building, and crises incidents through individual meetings with staff and participation in team meetings._
- _ Support the promotion/awareness building of the programs and its services to the immediate and broader community._
- _ Support in the development of and implement program initiatives, such as food access, peer involvement, and social and recreational and life skills programming._
- _ Through ongoing consultation with community agencies and development of a close collaborative partnership of all local providers, actively build a continuum of care for seniors with mental health and addiction issues._
- _ Provide consultation to the team and other service providers on concurrent disorders and harm reduction, including offering programming on these topics for the participants._
- _ Support in the development of common strategies to address tenancy and building issues as they arise._
- _ Support WoodGreen Housing and Building Services with property management at Cedarvale._
- _ Support the Housing Unit with tenancy issues, including pest control, hoarding, arrears, inspections, notices and work orders._
- _ Prepare statistical and program reports for the Manager as requested._
- _ Ensure that the equity objectives and community development policies of the organization are followed within the programs._
- _ Participate in regular supervisory sessions with the Supervisor and or Manager._
- _ Engage in training and education to upgrade skills as directed._

What You Bring to the Team_**
- _
Bachelor’s of Social Work or Bachelor’s degree in a relevant field and equivalent experience._
- _ 3+ years of experience implementing programs; working with homeless and vulnerably housed individuals, and with older adults and seniors. _
- _Knowledge of RTA and occupancy agreements, and property management._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._
