**POSITION PROFILE**:
Antimicrobial Stewardship Pharmacotherapy Specialist partners the program to establish standard guidelines that ensure appropriate antimicrobial therapy. This includes: antibiotic prophylaxis, establishing guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy associated with a definite diagnosis, optimizing antimicrobial dose and route of administration based on pharmacokinetics and pharmacodynamic principles.

**QUALIFICATIONS**:

- At a minimum, completion of a Bachelor of Science Degree in Pharmacy (BScPhm) or completion of Doctor of Pharmacy degree (PharmD);
- Certificate of competency from the Pharmacy Examination Board of Canada, required;
- Active registration with the Ontario College of Pharmacists, required;
- Three (3) to five (5) years of related experience, required;
- Advanced clinical degree with infectious disease training or equivalent clinical experience;
- Demonstrated experience in preparing and delivering individual and group education sessions;
- Excellent customer service skills;
- Exercise initiative and good judgement with ability to multi-task;
- Demonstrated superior interpersonal and communication skills, both written and verbal;
- Strong organizational and time management skills required;
- High proficiency in conducting research, compiling information and maintaining records;
- Computer skills and familiarity with multiple software programs;
- Ability to establish effective working relationship with others across disciplines and departments;
- Ability to work independently with mínimal supervision;
- Knowledge of applicable legislative, UHN and/or departmental policies;
- Compliance with confidentiality requirements;
