**CARE COORDINATOR**

- **I. The Organizations**_
- Canadian Religious Stewardship _(CRS) is a not-for-profit charitable organization established by Catholic Religious Institutes from across Canada. CRS is assisting Catholic Religious Sisters and Priests based at Presentation Manor for Seniors in Scarborough Ontario with their need for a Care Coordinator.

The Sisters and Priests live together in community with other religious and lay persons at Presentation Manor for Seniors and at Providence Healthcare in Scarborough, Ontario.
- **II. The Position**_

Working collaboratively with the Care Navigator of CRS and in close coordination with the Director of Care at Presentation Manor for Seniors, the Care Coordinator will provide planned, patient-oriented nursing care in accordance with the provincial standards of the care of CRS clients at Presentation Manor for Seniors. This extends to serve Religious in independent and assisted living, as well as liaising with Medical Allied Health Professionals for those Religious living in long-term care at Providence Healthcare. This role aims to complement existing healthcare services in place, requiring excellent communication skills.

This is a permanent position that requires alternating weekend shifts, and may occasionally necessitate responding to emergencies, highlighting the professional nature of the role. The position requires a sense of responsibility and readiness to be available for urgent situations as they arise. Any adjustments to the employee's hours of work will be agreed upon between the employee and the employer.
- **III. Requirements**_
- Certification of competence from the College of Nurses of Ontario (CNO)
- Successful completion of RPN medication administration course or equivalent
- Five years of health care experience
- Excellent knowledge of, and ability to navigate the Ontario health care system
- Valid Basic Life Support (BLS) Cardiopulmonary resuscitation (CPR) Level C certificate and First Aid certification
- Valid driver's license
- Physically fit to perform duties
- Available to work alternating weekend shifts
- Willingness to, as a professional, occasionally respond to emergencies as they arise
- Vulnerable Sector Police Check
- **IV. Key Responsibilities**_

Key responsibilities include, but are not limited to:
Collaboration and Care Planning:

- Collaborate with residents, CRS, and Presentation Manor for Seniors staff
- Assist Religious in goal setting and identify strategies for desired outcomes
- Work in partnership with each member of the Religious community and the care team to create individual care and service plans aligned with identified priorities and desired outcomes
- Determine eligibility for, arrange for services, and refer patients
- Lead, coach, mentor, and supervise Care Companion(s) and RPNs
- Complete other duties as required working in collaboration with the CRS Care Navigator

Communication and Coordination:

- Foster communication with Presentation Manor for Seniors' care team members, including representatives of the congregations (e.g., health committee and animator), Providence Healthcare, physicians, specialists, and other healthcare professionals such as CareRx Pharmacy
- Coordinate vendor and resource utilization involving medical equipment and supplies
- Respond to resident concerns and ensure appropriate action taken and or concern communicated to the Care Navigator and other departments
- Collaborate with the Care Navigator and staff in the coordination of Infection Prevention and Control, Health and Safety, and Clinical Programs.

Resident Support and Well-being:

- Ensure regular visits to Religious
- Schedule and accompany Religious to medical appointments, including Emergency Department visits as required
- Process the orders and safe keeping of drugs and narcotics and administer in compliance with all regulatory requirements, scope of practice and policies and procedures
- Assess understanding of, and learning needs related to diagnosis, treatment, available resources, illness adjustment, and coping mechanisms
- Facilitate understanding and collaboration among members of the Religious and their multidisciplinary care teams
- Advocate for Religious health care needs
- Promote physical, intellectual, emotional, psychosocial, and spiritual well-being of Religious

Documentation and Compliance:

- Maintain detailed and accurate client documentation
- Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate and up to date and changes communicated as required
