**Care Coordinator - Nurse Next Door Home Care Services**

**Position**: Care Coordinator
**Hours**: 9:00 AM - 9:00 PM
**Location**: Remote or Local Office (with in-person visits required)

**About Nurse Next Door Home Care Services**
Nurse Next Door provides 24/7 support to clients and their families, offering a wide range of home care services that bring peace of mind and support for daily living. As a Care Coordinator, you’ll ensure seamless care by managing schedules, addressing client inquiries, and coordinating with our healthcare team. This position requires a nursing background to assess and meet client needs effectively.
**Key Responsibilities**

- **Primary Client Contact**: Act as the main point of contact for clients and families, ensuring a responsive and compassionate experience.
- **In-Person Client Meetings**: Visit clients as needed to assess care needs, build relationships, and provide updates to healthcare providers.
- **Effective Communication**: Work closely with healthcare providers, clients, and families to ensure a high level of service.
- **Timely and Compassionate Support**: Address inquiries and concerns with empathy, providing comprehensive solutions.
- **Collaborate with Team**: Work with the Nurse Next Door team to optimize client care and improve service quality.

**Qualifications**
- **Nursing Background**: Experience in healthcare, with a nursing background to ensure high-quality client care.
- **Driver’s License**: Required for in-person client meetings and assessments.
- **Experience with AlayaCare**: Familiarity with AlayaCare software for scheduling and care coordination.
- **Strong Communication and Organizational Skills**: Ability to effectively engage with clients, families, and the healthcare team.
- **Client-Focused and Detail-Oriented**: Compassionate, attentive, and independent, with a focus on quality care and discretion.
**Nurse Next Door Home Care Services - Making Lives Better, One Visit at a Time.**
**Job Types**: Full-time, Part-time
Pay: $16.55-$40.89 per hour
Expected hours: 36 per week
Additional pay:
