Social Worker

Primary role is providing support to residents and their families to achieve optimal social functioning.
**Responsibilities**:
- Provide on
- site individualized resident assessment, psychological and holistic support and counseling for residents on an as needed basis.
- Provide support during admission, stay at LTC home and discharge as requested;
- Conduct initial assessment of resident, proposed interventions and progress in treatment and document as required using home’s electronic documentation record;
- Develop a plan of action within 24-48 hours of receiving a referral from interdisciplinary team; identify appropriate interventions and/or discharge plan and document in residents plan of care;
- Provide consultation and interventions to resident and family members as requested to ensure resident health and well being and quality of life goals are identified, enhanced and maintained;
- Assess, plan, implement and evaluate resident goals identified;
- Coordinate and liaise with community services and LTC Manager of Resident Care
- Attend care care conferences as requested
- Comply with Long Term Care Act & Regulations, MFIPPA, etc.
**Reports** to LTC Home’s Manager of Resident Care
**Qualifications**
- Social Service Worker Diploma
- Current registration in good standing as practicing Social Service Worker with the Ontario College of Social Workers and Social Service Workers
- Minimum of 1-2 years recent experience in long term care is preferred
- Knowledge and experience in gerontology as well as an understanding of the needs of the long term care clients, RAI MDS and Point-click Care documentation.

Licence/Certification:
- Degree in Social Work (BSW) (required)
- RSW license from Ontario College of Social Workers (required)
