We are looking for a
**Clinical Practice Specialist-**
**Learning and Development** to provide one on one care in the community/home setting as part of our interdisciplinary care team.

What The Role Involves:
**Advances Standards of Care and Professional Practice**
- Champions a culture that is committed to and passionate about patient-centered, high-quality and integrated care and the identification of transformative opportunities in enabling point-of-care excellence in every patient and caregiver interaction
- Collaborates with interdisciplinary patient care teams, operations and quality and professional practice leadership supporting the implementation of local, regional and provincial strategies that will advance excellence in Community Support outcomes from a population health lens.
- Continually updates personal knowledge regarding current best practices regarding PSW services, trends in community care, College of Nurses of Ontario (CNO) standards and guidelines, and adult education methods/strategies
- Works closely with Directors, QPP team, Nurse Managers, and Personal Support Managers to identify common educational needs, develop common solutions, implement educational sessions and circulate resources both for nurses/CSWs education and patient/family education.
- Inspires Personal Support Managers to seek out knowledge independently and to continually improve in the performance of their role
- Develops and implements standardized orientation for newly hired PSMs, and CSWs, including:

- Continually updating and circulating revised CSW orientation in response to branch feedback and organizational/industry changes
- Training local branches in the delivery of CSW orientation
- Evaluating delivery of CSW orientation at local branches and addressing deviations from standards.
- Develops and implements patient assessment and care documentation system for CSWs and Personal Support Managers in accordance with CNO standards
- Supports CarePartners’ strategic priorities relevant to professional practice and ensures leadership is informed of emerging trends in technology, health care, and adult education for CSW's and Personal Support Managers
- Supports the IPAC program in the absence of the manager of IPAC

**Practice Analyst and Advice**- Collaborates with the local CarePartners branches to identify issues and trends regarding practice gaps and develops proposal, in collaboration with leadership, to mitigate activities and ensure alignment with high-quality patient-centred practice and programming expectations
- Provide strategic advice on CSW practice and programming issues and themes

**Build Capability and Education**
- As part of the program of Personal Support Manager orientation, provides orientation to each newly hired Personal Support Manager in:

- Special Functions (face to face or virtually)
- Documentation of patient assessment and care
- ** Dementia Care**
- Provide consultancy service and support to leadership and frontline staff for the promotion of practice and programming excellence, in line with CarePartners internal and regulatory standards
- Supports the consistency of CSW care practices across the branches
- Works closely with Patient Care and Operations leadership and collaborates on clinical orientation/role developments
- Champions the use of digital learning platforms and identifies infrastructure necessary to enable nimble and responsive learning for frontline team members
