**This position starts on November 1, 2023 and will end on May 1, 2025, with the possibility of extension.**

**Hours**:40 hours per week; shift work; flexibility is required as times may vary depending on the needs of the program. **Some evenings and weekends will be required.**

**Responsibilities include**:
¨ Acting as a strong advocate for the individuals in the community

¨ Assisting individuals with daily living skills and finding employment

¨ Assisting with substance abuse issues using a Harm Reduction approach

¨ Assisting with managing mental health, substance use, health issues and trauma with other community partners in a multidisciplinary team

¨ Behavior Management/supporting adults with complex needs; support with legal matters

¨ Program implementation and development

¨ Case management, including maintaining and updating working files

¨ Completion of daily documentation and data collection

¨ Working with individuals and staff team to create, implement, maintain and update personal support plans

¨ One (1) year of post-secondary education in a related field

¨ One (1) year of related experience in the Human Services field with **demonstrated case management skills**

¨ Equivalencies may be considered

¨ Security Clearance Check; including Vulnerable Persons Sector Check (within the last 6 months)

¨ Emergency First Aid—Level A CPR + AED

¨ Valid Class 5 drivers license, drivers abstract, reliable vehicle and auto insurance **are required**

¨ Familiarity with working with high risk populations, addictions, poverty, homelessness and the justice system

¨ Ability to work independently

¨ Ability to build healthy relationships and manage crisis situations

¨ Excellent verbal and written communication skills; computer/word processing skills

**Starting Rate of Pay**:$27.03 - $28.68 per hour; depending on experience and education
