We are looking for a** Full-Time Home Share/Community Inclusion Coordinator. **This role is responsible for the administration of the Home Share and Community Inclusion (CI) programs including approving and implementing new provider and client contracts in accordance with government and Agency requirements, authorizing the funding for contract pay-outs, resolving cross-program and strategic issues with stakeholders, and overseeing staff. This position is **Monday-Friday, 40 hours/week. **Some on-call duties may be required

**You must have**:

- Bachelor's degree in the social services field or equivalent education and experience
- Minimum 5 years’ experience in a social services role
- Minimum 1 year of experience in a supervisory role
- Superior relationship management skills and experience interacting with contractors and stakeholders
- Clear and effective verbal and written communication skills
- Ability to pass a Criminal Record Check
- Current First Aid and CPR certification
- Current Non-Violent Crisis Intervention certification
- Ability to travel frequently to different locations in the Lower Mainland

**You’ll score more points if you have**:

- Taken Community Living of BC/Justice Institute of BC Home Study course
- A strong understanding of contract principles, program governance, and case management
- Knowledge/experience with the Home Share and CI programs

**A (small) sample of what you’ll be doing**:

- Program_ _Responsibilities_
- Implement the Home Share program based on requirements mandated by Community Living of BC
- Develop new programs as needed and recruit and approve new Home Share providers
- Manage a training program for Home Share providers with regards to contract implementation, service delivery, and evaluation of services provided
- Case Management _Responsibilities_
- Facilitate and participate in meetings with health care professionals, clients, families, and home share providers to understand and meet client needs
- Engage in client and home share provider visits to perform environmental assessments and service evaluations
- Manage risk issues and implement corrective measures
- Administration_
- Approve the department's staff schedule and enter time worked in ADP Workforce Now
- Manage the recruitment and development of staff including coaching, training, and performance assessments
