We are looking for a Community Health Manager to join our team in North Vancouver.

The **Community Health Manager (CHM)** role reports directly to the President & CEO, and is responsible for successfully building relationships in the community attending networking events, meeting with referral partners and healthcare facilities, and providing exceptional client service and caregiver management.

The CHM will be empowered to be themselves and put their heart on their sleeves. This will empower them to show up as their best self each day. Development, support, and feedback will be provided from a number of professionals on an ongoing basis including being part of a network of leaders with similar roles from regions across Canada.

Community Health Manager Main Responsibilities:

- Community, Referral Partner and Facility Relationship Management and Lead Generation: Proactively uncover, develop and close new business opportunities
- Securing and onboarding new Clients: Upholding JLF values and standards, successfully handle first contact through to client visit and caregiver introduction
- Onboarding and Retention of Caregivers: Successful onboarding and retention of caregivers to fulfill current and anticipated/forecasted client needs

- Ability to handle difficult situations and if needed to pivot.
- Self starter with a positive, can-do attitude, possess a curiosity and willingness to learn
- Valid Driver's license, and use of own vehicle
- Sales and marketing experience an asset
