**Responsibilities**:
Reporting to the Public Health Early Years Manager, the Community Home Visitor (CHV) works in collaboration with the Early Years Consultant and others as appropriate to ensure Early Years programs and services are planned and implemented to address local population health needs.

The CHV works with at risk families and vulnerable populations during the prenatal, postpartum, and early childhood period. The CHV implements the Healthy Beginnings Enhanced Home Visiting program including strengths based curriculum for at risk families. The CHV is part of a collaborative multidisciplinary team approach and works with the Early Years Team to ensure quality case management. The CHV shares a core set of public health attitudes and values, applies public health core competencies to all of his/her work and is accountable to provide competent, safe and ethical professional practice. The CHV ensures that the principles and practices that guide the Nova Scotia Health such as the vision, mission, values, strategic directions, ethics, safety, quality, partnerships and inter-professional collaboration are integrated within the work that they do and through the messages they deliver.

**Qualifications**:

- Grade XII and diploma in Early Childhood Education or Human Services from an accredited education institution required
- A valid driver's license and access to a reliable vehicle, travel will be required
- Cultural First Nation Competency Training an asset
- Current CPR and First Aid Certification
- Computer proficiency with Microsoft Office programs
- Ability to use audio visual equipment and general office equipment
- Demonstrates the core competencies for public health as outlined in the job analysis
- Demonstrates knowledge in the Health Protection Act and other relevant Regulations, Legislation & Acts, specifically the Child and Family Services Act
- Demonstrated ability to communicate effectively, including team building, negotiation, conflict management
- Demonstrated ability to work cooperatively and collaboratively in a multi-disciplinary setting
- Demonstrated accountability and responsibility
- Demonstrated ability to plan and be self-directed

**Hours of Work**:

- Permanent, Full-Time Position; 70 Hours Bi-Weekly
- Frequent Travel will be required

**Salary Information**:
$21.61 - $24.55 hourly
