- The Disease Pathway Management (DPM) Program facilitates an integrated and fulsome approach to system-level disease site oversight. It is a unique program with initiatives that span the cancer care continuum (e.g. prevention to recovery and end-of-life care). The program partners widely across Ontario Health by advising on disease specific issues, identifying gaps in care by each cancer disease site (e.g. Breast, Gastrointestinal, Thoracic, etc.) and fostering quality improvement through a variety of methods (development of guidelines, pathway maps, educational presentations, etc.). There are 8 disease site portfolios composed of a Ontario Cancer Lead (clinical lead) and coordinated by a Specialist or Senior Specialist.
- Reporting to the Manager, the Team Lead, will be responsible for:
- Overseeing projects that address healthcare gaps and facilitate quality improvement. This includes working closely with various Ontario Health programs, and clinical experts on stakeholder consultations, literature reviews, needs assessments / environmental scans and communication/education materials-
- Effectively managing, coaching, and mentoring the team, promoting collaboration and an enjoyable team atmosphere with high levels of engagement and morale-
- Fostering strategic relationships with clinicians, health service providers, and other key stakeholders to identify care gaps as well as facilitate and drive quality improvement opportunities-
- Leading complex initiatives and engagement across multiple areas that may encompass the Cancer Programs Portfolio-
- Supporting the Manager in issues management and communications (i.e., can act as a delegate/effectively provide coverage or independent support)- Here is what you will be doing:
- Manage the Disease Pathway Management Program team (1 Senior Specialist, 2 Specialists) who facilitate disease specific portfolios which are led by the Ontario Cancer Leads (8 disease specific clinical leads), including:- Plan and oversee DPP deliverables to ensure they are of high quality, clear, accurate, succinct, and timely. This includes reviewing and editing communications, reports, presentations and other project materials-
- Ensure structures are in place to optimize communications and processes across disease portfolios (e.g. Consistent approach to disease pathways, horizon scanning, etc.)-
- Build strong engagement and collaboration among team members; facilitate employee communications & engagement-
- Mentor and provide opportunities for team to improve upon and expand skills, with a focus on ongoing operational refinements and the identification of improvement opportunities-
- Ensure clinical lead engagement though procurement and performance management processes-
- Support the DPP team in issues management-
- Lead initiatives that cross disease portfolios / programs to advance quality care gaps in patient care to identification of emerging evidence and changes of practice to implementation including policy development, reports, etc. (e.g., environmental/horizon scanning, knowledge translation, etc.)Liaise with key internal and external stakeholders, including other Ontario Health program, clinicians, and health administrators-
- Lead or support development of varied communications and documents, including briefing notes, clinically
- and/or health administrator-focused communications, presentations and business cases, manuscripts-
- Contribute to the development of measurement strategies and perform ongoing review of program indicators & analysis-
- Draft and coordinate timely, clear, accurate and concise responses to Ministry, media and patient inquires-
- Identify risks and issues, escalate to Manager, and work with Manager on resolutions, as needed- Here is what you will need to be successful:
Education and Experience- Master’s degree required in an appropriate discipline (clinical epidemiology, health services research, business/health administration, sciences) or equivalent combination of education and experienceKnowledge and Skills- Minimum of eight (8) years of overall working experience, with a minimum of five (5) years recent experience working within the healthcare sector-
- Minimum of two (2) years in a team mentoring/leadership role-
- Strong knowledge of the cancer care system and/or clinical/hospital administration processes-
- Good understanding of clinical concepts, terminology and evidence-based decision-making-
- Demonstrated superior stakeholder relations skills including the ability to develop and maintain collaborative working relationships, build credibility, consult, influence, persuade, build consensus, coach, and resolve conflicts; experience working with clinical experts and health care administrators-
- Proven track record leading/enabling systems and clinical level strategic projects-
