**Admissions Coordinator**
**Compensation: $20.79/Hour
**The Role**
Reporting to the Client Services Manager
- Adult Day Program, you will complete the intake of referrals effectively and efficiently from Home and Community Care Services into the Adult Day Program
- This is a temporary full-time position, 35 hours per week, 8:30am - 4:30pm - Monday to Friday with occasional work outside of business hours. Some travel required. _
**Key Responsibilities include but not limited to**:
- Coordination and facilitation of effective and timely admissions to various Adult Day Program locations
- Liaison with CHATS departments, other care agencies, and families to ensure a seamless continuum of care with clients.
**What you bring to the role**:
- Two years of direct customer service experience with frail, older adults or an equivalent combination of training and experience.
- Knowledge of senior-related health conditions
- Strong knowledge of health and community support systems and resources
- Strong communication, telephone, and customer service skills
- Strong organizational skills
- A vehicle and valid Ontario Driver’s License are required.
- Knowledge of diverse cultures an asset
- Fluency in both official languages (English or French) or other languages is an asset.
