**Caregiver Education Counselor (CEC) and Alternative Level of Care (ALC) Coordinator**

**The Role**:
Reporting to the Client Services Manager, Community Programs, you will have a dual role, delivering direct service and supporting program operations.

You will be involved in client care needs assessment (independently or with a partner agency), provide care coordination, liaise with relevant CHATS program supervisors and coordinators and serve as the primary contact for CHATS for the OHT initiative and to provide Caregiver Support and Education within the scope of the Hospital to Home program.

This position has an emphasis on addressing hospital pressures and a focus on reducing ALC days and repeat ED admissions through planning and support robust transitions to community.

This is a full-time position, 35 hours per week, Monday to Friday with occasional work outside of business hours.

**What you bring to the role**:

- You have a desire to transform care and be leading edge within the healthcare system. You will work within an integrated health care environment and possess, excellent assessment, communication, case management, organizational and problem-solving skills.
- In addition, you have expertise and experience working with older adults with dementia or complex medical conditions and their caregivers.
- Regulated Health Professional or Degree in Gerontology preferred, or other relevant education or experience providing senior and/or caregiver support services in the community.
- Fluency in both official language (English & French) and/or other language(s) will be considered an asset.
- Ability to accommodate a flexible work schedule including evenings and weekends.
- Current certification in First Aid and CPR.
- Excellent skills in risk, crisis and conflict management.
- Must be registered with the College of Social Service workers.
