**Job Overview**

Under the supervision of Health Director, the Community Health Representative will work in partnership with Health Department Staff with the provision of screening programs; health education and communicable disease control, and enhance the community capacity through strong community partnerships.

**Responsibilities**
- Raise awareness on and provide education on disease prevention
- Promoting awareness and assisting in linking clients, families, and communities to local social service agencies
- Participating in the provision of health promotion and disease/injury prevention programs through partnerships, which focus on self-reliance and self-determination.
- Assist Nursing staff with obtaining information to better understand the client and family health concerns, and barriers to access the health care system.
- Developing culturally appropriate educational programs and materials to promote health in the community.
- Provide safety and health education to individuals and their families and other duties as assigned.

**Requirements**:

- HIgh School Diploma or GED equivalent.
- College certificate/diploma in related health care discipline, such as Community Health Representative, Social Work, or Nursing preferred (If part of a registered profession-registration in good standing with applicable governing body)
- Evidence of strong interpersonal and communication skills.
- Criminal record check with vulnerable sector check required
- Must be able to take part in developing and maintaining respectful professional working relationships with team members, clients, families, and the community.
- Current valid Alberta Drivers License
- Previous experience in an Aboriginal organization setting would be an asset
- Ability to work independently as well as in a team, good verbal, and written communication skills
- Excellent customer service focus, with an ability to work collaboratively with others
