**KEY FUNCTION**:
Complete screening tools to assist with minimization of length of stay. Collect and analyze data to determine outcomes.

**REPORTING**:
Under the general direction of the Manager, System Transformation.

**DUTIES**:

- Engage patients in intial Home First Philosophy conversation.
- Meet with patients to complete screening tools used to assess risk for an extended length of stay.
- Determine priority of focus in order to achieve an estimated date of discharge (Clinical, social, functional).
- In collaboration with a regulated health professional, review completed screening tool(s), assess results and make recommendations on service required based on score.
- Act as a point of contact for departmental staff, other departments, programs, and community stakeholders as required.
- Attend daily meetings (Emergency department huddles, bed rounds and bullet rounds) and be aware of daily bed pressures.
- Communicate across care team and engage with community partners effectively.
- Develop strong relationships with physicians and interdisciplinary team members in the Emergency department (ED) and across the organization.
- Document score and action taken/required on appropriate form and attach to the patient chart.
- Provide support for patients, caregivers and informal supporters being served by the organization and community agencies.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a Bachelors degree in health related field or equivalent.
- Training in Lean methodologies is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of two (2) years’ experience working in a within a health care environment.
- Experience working with or knowledge of assessment tools (i.e. InterRAI, Barthel Index, Blaylock, etc.) is preferred.
- Experience working with the elderly and their care givers/informal supporters is preferred.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Ability to work independently and/or as part of a team.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated excellent presentation skills.
- Demonstrated knowledge of Medical Terminology.
- Demonstrated excellent judgment and proven analytical skills.
- Demonstrated ability to mentor students and support personnel.
- Demonstrated knowledge of current trends in general health care and community resources.
