**Position Description**:
The **International Case Coordinator** for the International Patient Program is accountable for providing ongoing support and expertise through assisting in timely comprehensive assessment, planning and overall evaluation of individual patient needs.

In this position, you will follow the regulatory guidelines set by the Ontario Ministry of Health and Long-Term Care and the Canadian Medical Protective Association (CMPA). You will review medical documentation and determine the appropriate clinical stream for the patient by working closely with those clinical areas and programs within SickKids that have additional internal capacity to accept international patients. You will liaise with external referring partners (ie. Pediatricians, family doctors and other medical specialists, insurance companies and embassies) to ensure appropriate information is collected regarding the patient clinical care needs and family situation.

**Here’s What You’ll Get To Do**
- Lead intake evaluation, assisting in development and revision of individual treatment plans by collaborating with the clinical team, participation in clinical care planning by attending case conferences and rounds
- Provide information to patients and families on the intake process and international patient service offerings
- Provide family-centered case management and discharge/disposition planning through coordinating internal and external resources for each patient and working with the clinical team to determine appropriate follow up and discharge from treatment
- Provide guidance to the Program Coordinator to ensure timely coordination and triage of all referrals and collaborate on data collection and data management
- Understand clinical team functions, roles and responsibilities within programs delivering care to international patient and families
- Support patients with navigating access to healthcare at SickKids, understanding immigration and transportation requirements
- In conjunction with members of the clinical team, provide recommendations on referral of appropriate services required abroad to facilitate transition and discharge to home country
- During periods of high intake patient volumes and as an expert in intake, may be requested provide direct support to Program Coordinator
- Demonstrate initiative in leading and/or contributing to quality and process improvement projects

**Here's What You'll Need**
- BScN or BSc in a health-related field
- Minimum 5 years of experience in a health-related field
- Experience in working with international projects, case coordination and knowledge of international traditions, laws, and core cultural values, as required when working with international agencies, healthcare professionals and families

**Employment Type**: 15 month full-time contract; includes benefits & personal days
