**Fife House** is an innovative, client-focused provider of secure and supportive affordable housing and services to people living with HIV/AIDS in the Greater Toronto Area. With approximately 110 staff working across 6 locations, in 2020-2021,Fife House served more than 850 residents and clients by providing supportive and transitional housing programs as well outreach programs to homeless clients and their families.

**Fife House** is seeking a dedicated individual to fill the role of Residential Support Worker positions across a number of our programs. As a member of the team, the Residential Support Worker will provide front-line client-centred and trauma-informed care to Fife House residents/clients across service locations, in alignment with unique program needs and fulfillment of relevant funding deliverables.

**Highlights of the Role**
The position is responsible for providing high quality residential support for 45 HIV+ clients (elderly, adults and youth) who are gender fluid, non
- binary and belong to the BIPOC community. The program is committed to fostering an environment which promotes safety and community for persons living with HIV who have or are currently experiencing traumatic events, abuse, substance use/ addictions, mental health disorders, homelessness, parental/family conflict, neglect and or issues with attachment. This position provides direct care and case management, as well as administrative and household functions of the residence.

**DUTIES AND RESPONSIBILITIES**:
**Client centered approach** - direct client-contact in all aspects of service necessary for the comfort and safety of clients at Sherbourne program.

**Service planning and Care Supports** - works collaboratively with residents (including children) in the areas of housing, legal. Education, employment and mental health to establish and maintain a plan assist them with a safe & effective path at 490.
- Develop specialized plans of care with individualized goals for residents. Including, but not limited to medication adherence plan, Assisted daily living plans, safety plans and discharge plans.
- Complete yearly CHA reports with clients and update health file on digital platform.
- Provide case management support to client based on service plan and IPCC.
- Update consent, go to hospital forms, face sheets and circle of care documents.
- Provide case management progress reports to management during team meetings.
- **Providing supports to the residents - **Assess, investigate, resolve any concerns or issues of our diverse and often vulnerable tenant population.
- **Documentation & Report Writing**
- Update client records such as: OCASE, Client Record Management Drive, intake/ discharge, correspondence, CHA (InterRAI Assessment), consent/ release information and Client service plans.
- Completes shift logs, occurrence reports, intakes and discharges.

**Collaboration with team & community partners** - Participate in weekday programs such as life skills, outings, games and other activities.
- Maintain professional and appropriate verbal communication with co-workers, on-call counselors, management, clients and members of the public while on duty
- Actively participate in community and in-house initiatives, multi-agency networking and case conference meetings to ensure effective service delivery for hard to serve individuals.
- Works with student placement and community navigators.
- Works with general rules, procedures and standards, referring issues or problems to program Manager
- Provide support through in-person and phone contact with circle of care, informal counselling, treatment updates and recommending services and supports.

**Maintain daily living of residents** - Supports residents in maintaining residency, including yearly taxes, income review assessment, hoarding, eviction prevention
- Completes in home visits with individuals and or support circle members to assess and implement care plans.
- Respond to crisis, emergencies or safety concerns with clients and conduct check ins of tenant needs and safety plans
