Patient, Family and Care Partner Co-design Lead

**BWH Required Skills**:

The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

**Principal Duties**:
Community Engagement
- Assist S-L OHT to identify gaps and opportunities for improvement in areas where the patients' and caregivers’ perspectives should be incorporated
- Work collaboratively with the S-L OHT Engagement and Partnerships Lead to provide advice and assist in engaging and communicating with diverse community groups
- Help with the recruitment of new patient, client, care partners and building the network of community feedback
Operational Responsibilities
- Regularly attend various meetings as needed; participate, inquire, and share relevant insight while representing the interests, and needs of S-L OHT patient, family, and care partners
- Maintain confidentiality, as appropriate, by not discussing meeting information outside of the meetings
- Provide input to creating meeting agendas and prepare for meetings by reviewing supporting documentation
- Work collaboratively with the other Co-Chairs, Leadership Team members, and the Engagement and Partnerships Lead, providing updates, ideas, and information
- Contribute to developing applicable S-L OHT policies and priorities, and participate in planning and designing the activities of S-L OHT - Assist in the development and execution of the PFCPAC (Patient, Family Care Partner Advisory Council) Work Plan
- Assist with interviews and PFCP orientation and onboarding
- Participate in completing applicable reports to Ministry of Health and Ontario Health as they relate to Patient, Family and Care Partner Engagement
Communication Responsibilities
- Serve as a S-L OHT resource for community members and staff, and serve as an informal community ambassador
- Promote the S-L OHT PFCP advisory council work to various internal and external audiences as needed, to increase visibility of the PFCP Advisory Council as a community engagement resource
