Patient Flow Coordinator

**BWH Required Skills**:
The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

**Qualifications**:
- Regulated Health Professional required
- Bachelor’s degree in a health related discipline preferred
- BSCN preferred
- BCLS preferred
**Principal Duties**:
- Provides support, guidance and mentorship in terms of decision making around patient flow, staffing, and hospital processes
- Coordinates all repatriations from referring facilities and supports BWH in life and limb policy
- Supports and facilitates patient flow directed discussion through Medworxx and Oculys patient flow tools
- Facilitates daily bed meetings to support patient flow throughout the organization, reporting back to BWH Leadership team
- Collaborates with community partners to develop discharge plans and transitions to community/home
- Provides administrative and clinical leadership to staff in a supportive, facilitative and empowering manner
- Ensures appropriate adherence to the bed management contingency/ full capacity protocol in order to maintain access to emergency services, meet the hospital’s commitments, and match patient needs with the appropriate environment and clinical support
- Participates in team building and promotion of good morale within the teams
- Provides supervision, direction, support, guidance, and/or education to clinical staff, as needed and in accordance with the policies, procedures, and guidelines established by Directors/Managers of each area
- Identifies areas requiring quality improvement; supports the process for identification of quality improvement initiatives related to patient flow, raises these issues at appropriate forums with recommendations for resolution
- Manages/facilitates the BWH Bed Management policy daily and after hours
- Accountable for following standard work processes of the patient flow coordinator
- Engages in situational management and conflict resolution strategies
- Ensures that all situations handled are documented and forwarded to the appropriate Director/Manager
- Supports and assists in the implementation of all strategic priorities
- Shifts may include on - weekdays and weekends 0800-1600 (weekend overnight on-call coverage), weekdays of 0800-2000 during periods of surge
- Facilitates daily bed meetings to support patient flow throughout the organization, reporting back to BWH Leadership team
- Other related duties as assigned
