Job Summary

We are seeking an experienced Nurse Practitioner to join our Palliative Consultative Team as a Clinical Care Manager. This is a full-time position with a hybrid work model, requiring travel between sites in the Erie St. Clair region.

Key Responsibilities

The successful candidate will be responsible for assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners. Key responsibilities include:

  • Evaluating patient eligibility for palliative care services
  • Developing and implementing individualized care plans
  • Coordinating care with community resources and healthcare teams
  • Providing education and support to patients and caregivers
Requirements

To be successful in this role, you must have:

  • A current registration with the College of Nurses of Ontario
  • Working knowledge of community resources and roles of healthcare professionals
  • Good knowledge of Ontario Health priorities, policies, practices, and service standards
  • Specialized education in palliative pain and symptom management
  • Excellent written and oral communication skills
  • Strong interpersonal skills with the ability to provide mentorship and maintain professional relationships
