**Job Purpose**:
The schedule of this position is as follows:
Wednesday: 8 AM - 4 PM
Thursday: 4 PM - 12 AM
Friday: 4 PM - 12 AM
Saturday: 4 PM - 12 AM
Sunday: 4 PM - 12 AM
Reporting to the On-Site Manager, the Housing Harm Reduction Worker will work with residents from diverse backgrounds who struggle with addictions and substance abuse issues. Through individual and group counselling sessions, the incumbent will work with identified individuals, using suitable intervention strategies and a harm reduction approach to assist residents in overcoming the challenges they face. Throughout this process, the incumbent will continually review, assess, and document residents’ progress and identify appropriate supports. The worker will be providing case management to an assigned case load in addition to providing general supports to all residents. They must collaborate in a warm and collegial fashion with team members including other employees, volunteers and clients, as well as, referral agencies to ensure successful and prolonged community integration and tenancy.
**Key Responsibilities**:
- Provide resident case management, individually and in group sessions, to assist in managing dependencies, adjusting to life, and making changes
- Complete and maintain accurate records and reports regarding the residents' histories and progress, services provided, and other required information
- Develop treatment plans based on research, clinical experience, and resident histories.
- Review and evaluate residents' progress in relation to measurable goals described in resident care plans
- Interview residents, review records, and confer with other staff in order to evaluate individuals' mental and physical condition, and to determine their immediate and longer term needs
- Intervene as advocate for residents in order to resolve emergency problems in crisis situations
- Provide residents with information about addiction issues and about available services and programs, making appropriate referrals when necessary
- Modify treatment plans to comply with changes in resident status
- Coordinate counseling efforts with mental health professionals and other health professionals such as doctors, nurses, and social workers
- Plan and implement follow-up and aftercare programs for residents to be discharged from treatment programs
- Conduct Harm Reduction orientation sessions
- Participate in case conferences and staff meetings
- Coordinate activities with community services and other agencies
- Instruct others in program methods, procedures, and functions
- Develop, implement, and evaluate public education, prevention, and health promotion programs, working in collaboration with organizations, institutions and communities
- Supervise and direct other workers, placement students and volunteers providing services to residents
- Monitors symptoms of addiction(s) and impact on functioning and continued community tenancy
- Performs routine finance and administration tasks including but not limited to: maintaining confidential files; completing forms; petty cash transactions
- Monitors the site and performs regular and frequent walk-around throughout the site to ensure smooth operations and safety
- Ensures the facility and surroundings are properly maintained, organized, and in working order
- Participates in ongoing training, learning, and development as required
- Performs additional tasks and special projects as required
**Required Skills and Competencies**:
- Must have a post-secondary education in social services, degree in social work, college diploma in social services with a specialty in mental health or equivalent experience
- Must have minimum two years or more work experience in the social services field
- Must have great awareness of harm reduction and recovery principles
- Have demonstrated experience in working with homeless and diverse populations
- Must possess superior interpersonal skills with a high level of social and emotional intelligence
- Must be aware of and display strong principles regarding client confidentiality
