**Position Overview**:
Reporting to the Manager of the Women & Children's Program and working as an active member of the inter-professional team on a fast-paced unit, the Social Worker will provide a range of services including assessment, crisis intervention, counselling and comprehensive discharge planning with individuals and families. Key components of the role include conducting psychosocial assessments and providing interventions based on a variety of issues such as child protection, mental health, domestic violence and perinatal/neonatal loss. Actively participating in regular team meetings, liaising with community based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed. The social worker acts as a valued member of the interdisciplinary team in order to see high risk prenatal patients, inpatients and follow up post discharge as required.

**Requirements**:

- Graduate from a recognized Master’s of Social Work Program
- Registered with the Ontario College of Social Workers and Social Service Workers
- Minimum 1 year prior experience in an acute care hospital setting
- Demonstrated knowledge of and experience with child protection issues
- Experience in individual and family counselling
- Experience working with teens and their families
- Experience in working with perinatal/neonatal loss and bereavement
- Experience with mental health issues, particularly related to pregnancy and childcare
- Excellent knowledge of community resources and liaison/referral experience
- Experience utilizing online documentation systems, Outlook, Excel
- Proven excellent interpersonal and communication skills, collaborating and negotiating effectively to achieve desired outcomes during the patient admission/discharge process
- Proven ability to work effectively as a member of a multidisciplinary team and independently
- Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
- Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
- Utilize protective equipment provided and identify and reports hazards
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
