Temporary Full-time Rehabilitation Worker Ii

Brockville, Canada Lanark Leeds Grenville Addictions and Mental Health Full time

In partnership with the United Counties of Leeds and Grenville, the Rehabilitation Worker II will work with the client on an individual Recovery Plan of Care to address client goals. The home and the environment will be used as a recovery rehabilitation tool to develop activities of daily living and promote client transition to a less intrusive and more independent living situation.

**Responsibilities and Duties**:

- Supervise client’s living in cooperative housing by promoting a healthy environmental milieu through facilitating skill development programs and psycho-educational groups for clients living in the home
- Develop and implement routines and programs to support skill development of clients in accordance to their recovery plan. Support clients moving to less supervised living accommodations
- Oversee the upkeep of the home to address health and safety of clients
- Assist in developing and maintaining clients budgets
- Encourage client participation in social recreation, vocation, education and wellness through the main office
- Monitor client compliance with medication regimes
- Provide direct skill development with clients either on an individual basis or with groups of clients in accordance with needs identified in personal recovery plan
- Participate in individual client Recovery Plan of Care team meetings
- Provide education to clients regarding home safety practices
- In consultation with case worker, assist client in attending scheduled appointments
- Facilitate regular house meetings for residents
- Respond to crisis situations as they arise

**Qualifications**:
**Education, Training and Experience**:

- Graduation from a recognized post-secondary institution in a relevant, health-related discipline of study.
- Two years’ experience working in mental health and addictions
- Course/workshop in concurrent disorders or equivalent (SMART Recovery Facilitator Training) preferred
- Certified Psychosocial Rehabilitation Recovery Practitioner (CPRRP) designation preferred
- Workshop in motivational interviewing preferred
- Demonstrated crisis intervention skills/experience
- Current First Aid and CPR certification required
- Satisfactory Vulnerable Sector police records check
- A valid Ontario Driver’s Licence, access to a vehicle, and vehicle insurance with a minimum of $2,000,000 liability.

**Skills and Abilities**:

- Understanding of Mental Health Act, Occupational Health & Safety Act, Ministry of Health and Long-Term Care guidelines, Psychosocial Rehabilitation Practitioner’s Code of Ethics, Best Practices, LLGAMH program standards
- Ability to work independently and interdependently
- Strong problem-solving abilities
- Requires analytical skills to gather and disseminate data from different sources
- Ability to adapt to changing environments
- Basic computer and keyboarding skills
- Compliance with Policy AH-C5 COVID-19 Vaccination Policy

**Job Types**: Full-time, Fixed term contract

**Salary**: $27.21-$28.92 per hour
