**Position Summary**:
Reporting to Manager, Community Mental Health Services and under the supervision of ACTT Team Leader, the Peer Support Specialist is responsible for offering a full range of client-centred clinical services in a facility and/or community setting. Services of the ACT Team are available 24 hours per day, 7 days a week and provided in accordance with the ACTT Program Standards, the Principles of Psychosocial Rehabilitation and the Mission, Vision and Values of BGH. The ACT Team Mental Health Peer Support Specialist works as a member of an interdisciplinary team independently carrying out treatment, rehabilitation and supports functions to facilitate community living, psychosocial rehabilitation and recovery within the context of the ACTT model for persons living with a serious mental illness. The Peer Support Specialist provides services to clients living with serious mental illness, enabling them to maintain an optimal level of functioning and independence. The Peer Support Specialist draws on their own skills and experiences as a consumer of mental health services to provide peer counseling, advocacy and side-by-side support to clients and families. The Peer Support Specialist also provides essential expertise and consultation to the entire team to promote a culture in which each client’s point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, and community self-help activities.

**REQUIREMENTS**:
**Education and Experience**:

- Direct experience and knowledge as a consumer of mental health services is required
- Two years’ experience providing services to adults with a serious mental illness
- Post-secondary education in Health, Social Services, Vocational Rehabilitation or Recreation preferred
- Regulated Health Professional (RN, RPN, OT, etc.)
- Previous experience working on an Assertive Community Treatment Team is preferred
- Ability to work both independently and as a member of an interprofessional treatment team
- Knowledge of local community resources including social, recreational, clinical and transportation services
- Current BLS and NVCI Certification
- Applied Suicide Intervention Skills Training
- Current Class “G” driver’s license and insurance and provide Driver’s Abstract and Criminal Reference Check

**Knowledge, Skills and Abilities**:

- Assist in the development and implementation of creative treatment plans, support services and programs responsive to client’s needs
- Demonstrated understanding of mental health symptoms, issues, psychotropic medications and associated side effects
- Provide direct assistance with activities of daily living
- Monitor client progress and modify treatment plans and services in consultation with the other members of the ACT Team
- Knowledge of the Mental Health Act, Personal Health Information Protection Act and other relevant legislation
- Knowledge of Assertive Community Treatment Team Standards
- Maintain appropriate documentation
- Provide assessment, advocacy and crisis intervention services
- Participate in the development and delivery of educational and information programs for employees, service providers, employers and the public
- Demonstrated commitment to fostering multicultural and 2SLGBTQ sensitive care environment
- Must be willing to drive long distances throughout South Lanark, Leeds-Grenville counties for client visits, clinics and community liaising
- Experience working in a computerized environment, sound knowledge and understanding of computer-based operations
- Excellent oral and written communication skills
- Excellent interpersonal and problem solving skills
- Excellent attendance and availability required
