**Work Hours**: 7.5 Hour Shifts Mon - Fri afternoons, evenings and weekends when needed

**Rate of Pay: $24.60**

**Job Summary**:
Gateway Residential and Community Support Services of Niagara is seeking one full time Street Outreach Worker to work with the Safe Opioid Supply with our Niagara Assertive Street Outreach (NASO). The Safer Opioid Supply (SOS) program provides clients using street-based drug supply, who are at high risk of overdose in the community and of medical complications due to drug use, a regulated supply of prescription opioids to reduce overdose and overdose deaths. The Street Outreach Worker will work within a harm reduction framework to provide support for SOS clients and work with the broader SOS team in providing wrap-around support for each client. The main focus of the SOS Outreach Worker will be to work within a harm reduction approach with the ultimate goal to secure permeant housing.

The Street Outreach Worker will establish a presence and build rapport with clients who are experiencing substance abuse, mental health and homelessness. The Street Outreach Worker will provide services and supports to clients with problematic substance abuse and use a harm reduction approach to assist them in focusing on finding permeant housing. Supports can include navigation of local services, supports, and opportunities that focus on recovery and overall well-being. The goal is for individuals who are experiencing homelessness and substance abuse, will be to create goals around their addictions and homelessness.

**Responsibilities**:

- Provides client-centered case management and coordination of services to overcome barriers to services and to promote an equitable treatment for people who use drugs
- Provides information and referral regarding community resources
- Identify which system areas SOS clients require case management support (CAS, PGT, OW/ODSP, criminal justice system, taxes, immigration, legal, housing, transportation, food security) and develop an action plan to address the social determinants of health that are impacting client health and wellbeing
- Leads case conferencing for SOS clients and follows up, with other team members of community and health system within PQWCHC as well as outside agencies and institutions.
- Street-based assertive outreach services engage individuals living rough with problematic substance abuse to secure permanent housing
- Creative; meaningful engagement with individuals who are homelessness and are struggling with addictions
- Assessment (including risk, needs, goals and strengths)
- Work with the clients to overcome the barriers surrounding, obtaining and maintaining housing
- Complete administrative tasks in a timely manner
- Work collaboratively with other providers on the SOS team including physicians, nurse practitioners, social workers, outreach workers, addiction counsellors, peer support workers, paramedics, and Supervised Consumption site staff to coordinate client care.
- Engage in collaborative community relationships to facilitate client access to services.
- Participate in Case planning / case conferences, implementation, evaluation, and review (to include relapse prevention, crisis resolution/contingency planning)
- Consideration for cultural and spiritual well-being
- Collaboration with substance abuse treatment and mental health providers
- Work in collaboration with the Niagara Assertive Outreach Team
- Use of HIFIS for record keeping / Mapping tool for encampment locations
- Vi-Spadat Assessments to monitor client progress
- Attend, participate in all team/staff meetings or training as required
- Connect clients quickly to the services they may need: using their knowledge and expertise of interacting with clients and their knowledge of what services are available in the Niagara Region
- Educate and support other workers on identifying individuals and supporting individuals with substance abuse

**Requirements**:

- Degree or diploma in Social Work or in Social Service Field
- At least one-year experience working with people who are experiencing substance abuse
- Knowledge of harm reduction principles is an asset
- Ability to de-escalate conflict is an asset
- Confidence in asserting and maintaining strong boundaries
- Experience in mental health and addictions
- Proof of full vaccination against COVID-19
- Experience and understanding of the homelessness sector
