Patient Transfer Coordinator

**The required training period is 12 to 13 weeks at approximately full-time capacity. If you’re interested to proceed, you must have flexibility to work shift work (up to 12 hours) including days, nights, weekends and statutory holidays in a 24/7 contact centre environment.**
The Patient Transfer Coordinator (PTC) works with stakeholders to support the movement of patients across the province to either a higher or lower level health care facility based on the patient’s needs and patient acuity, which can include out of province/country transfers. Works with regional ambulance dispatch centre to facilitate movement of patients between health care centres in the province. The PTC receives requests by methods such as telephone calls or online requests in a call center environment and processes patient transfer requests following online process maps and resources, referral pathways, and associated organizational policies and procedures. The PTC enters patient and case information into a computerized database, arranges teleconference calls between the sending and receiving physicians/care providers or delegate as required, also works in collaboration with receiving facility to confirm bed availability, and updates sending and receiving physicians as required. The PTC works in accordance with organizational standards, policies and procedures and participates in education provided by the Employer.
**What you’ll do**
- Receives patient transfer requests via telephone and online from stakeholders and collects required information, such as patient demographics and care requirements, to enter into a computerized database system.
- Carries out patient transfer requests in accordance with online process maps, resources and associated organizational policies and procedures. Processes requests in the computerized patient database system. Clarifies patient transfer requests between facilities and escalates as required. Remains familiar with provincial and regional referral pathways and associated protocols.
- Arranges teleconference calls between the sending and receiving physicians or delegate to facilitate transfer requests. Documents physician teleconference calls into the computerized database. Confirms with receiving health care facility regarding available bed options prior to transfers.
- Functions as a liaison by contacting patient flow and access teams at different health care facilities and exchanging related information regarding patient transfer requests to confirm bed availability, which can include out of province or country placements. Processes daily patient transfer assignments in order of acuity level, depending on transfer timeframes agreed upon by sending and receiving physicians.
- Updates sending and receiving physicians with regards to status/bed availability for pending patient transfers and arranges follow-up teleconference calls between the sending and receiving physicians to re-evaluate patient condition.
- Books transfer requests by obtaining transport care requirements through completing the form for BCEHS Patient Transport Coordination Centre (PTCC).
- Enters and/or updates data in accordance with standard procedures.
- Participates in quality improvement and advocates for patient centered care in patient care activities by identifying problems and/or inconsistencies, as well as opportunities for improvement, providing feedback and recommendations, and escalating issues to PTN Supervisor.
- Maintains working knowledge on established processes and procedures within BC Patient Transfer Network by reading notification of new and updated processes and procedures and participating in provided information/education sessions.
- Performs other duties as assigned.
**What you bring**
- Grade 12, medical terminology and graduation from a recognized Nursing Unit Clerk/ Medical Office Assistant program plus one year recent related experience or an equivalent combination of education, training and experience.
- Medical terminology is a must
- Flexibility to work extended shifts (up to 12 hours) including nights, weekends and statutory holidays in a 24/7 call centre environment.

- Ability to remain calm in a fast-paced environment.
