**ABOUT ACUMEN HEALTH**

Acumen Health, a leading Private Orthopedic Surgery Clinic and Reconditioning Centre in B.C, is seeking a dedicated individual to fill the role of Patient Care Coordinator. Reporting directly to the Clinic Manager and Operations Manager, this pivotal position requires a dynamic professional capable of seamlessly blending the responsibilities of a Patient Care Coordinator with those of a Medical Office Assistant, ensuring that all tasks at Acumen Health are executed with precision, efficiency, and utmost consideration for our clients.

This role is primarily remote, with in-person required as needed out of our Vancouver clinic locations.

**RESPONSIBILITIES**
- Perform data entry administrative tasks with accuracy and efficiency.
- Schedule patient appointments with our esteemed providers.
- Manage incoming calls to the clinic, providing comprehensive assistance and ensuring each patient receives the appropriate information and scheduling support.
- Uphold office policies and procedures to maintain organizational effectiveness.
- Extend a warm welcome to patients upon their arrival, overseeing the completion of paperwork and facilitating payment processing.
- Maintain meticulous patient records and ensure provider calendars are consistently updated and accurate.
- Coordinate patients' journeys from initial referral to surgical booking and post-operative care, ensuring seamless transitions at every stage.
- Collaborate with surgical centers and healthcare professionals across Canada.
- Conduct vitals assessments for joint replacement patients and schedule pre-operative assessments with family physicians.
- Provide patients with detailed information on surgical options and assist in booking surgeries.
- Arrange pre and post-operative appointments with our reconditioning team.
- Organize medical files for patient referrals, including laboratory and imaging reports.

**QUALIFICATIONS**
- Possess a professional and courteous demeanour, with excellent communication skills.
- Demonstrated organizational prowess to manage a high volume of detailed information effectively.
- Experience with CRM's such as Saleforce, Hubspot, Jira, or similar, is preferred.
- Ability to work both independently and collaboratively in a fast-paced environment with tight deadlines.
- Flexibility in scheduling is essential to accommodate the clinic's dynamic needs.
- A passion for learning and the ability to adapt to evolving tasks and procedures.
- Minimum 2 years of experience within a medical team setting preferred.
- Strong time and change management skills, coupled with a keen attention to detail.
- Exceptional phone etiquette and customer service skills.
- Genuine interest in educating patients about orthopedic options available in British Columbia.
