**Care Navigation Highlights**:

- Provides ongoing case management and support to clients/caregivers.
- Connecting clients to external resources, as appropriate.
- Has the necessary functional job knowledge to determine appropriate decisions and actions.
- **Connects** clients and caregivers to appropriate community support/resources.
- Provides assistance with navigating the health care system.
- Collaborates with partners and represents the agency externally at planning meetings.
- Implements plans and **makes** mid-course changes when necessary to achieve goals.
- Sets daily, weekly, monthly, quarterly, and annual project goals, creating specific plans to meet them.
- Is able to anticipate barriers to success and shows persistence in overcoming obstacles.
- Displays diversity awareness and sensitivity in verbal and written communications.
- Demonstrates tact and diplomacy in dealing with Clients, Caregivers, Stakeholders, Staff, and **volunteers**.

**Position Requirements**:

- Regulated Health Care Professional with Registration as a health or social work professional, including registered nurse, physiotherapist, occupational therapist, speech-language pathologist, or social worker. Registered Social Worker is an asset.
- A minimum of one year of experience within the community support service sector or health care environment.
- Experience working with the frail and elderly population, and knowledge of community resources including funding sources.
- **Knowledgeable** on the different levels of scams and preventative measures to keep older adults safe.
- Skills in navigating external organizations to access support to educate and bring awareness to older adults about using online banking, sharing passwords, using safe websites and sources, understanding phishing attempts, fake ads, and fraudulent websites.
- Excellent computer skills with proficiency in MS Word, Outlook, and PowerPoint.
- Have a Valid Ontario Driver’s License and access to a reliable vehicle.
- Valid Vulnerable Police Record check completed within the last 12 months.
- Current CPR/First Aid Certification.
- Fluency in other language(s) is an asset.
- **Mandatory COVID-19 vaccination required.**

**Job Type**: Fixed term contract
Contract length: 6 months

Pay: Up to $45,400.00 per year

**Experience**:

- RAI-CHA: 1 year (preferred)
- Community support service sector: 1 year (required)
