**Position Highlights**:
We currently have an exciting
**Temp Full Time** position as
**Lead, Health Career Access Program (HCAP). **In this role, you will lead the implementation of the HCAP program within the areas of Fraser Health owned & operated Long Term Care and affiliated/non-affiliated Long Term Care and Assisted Living sites.

This temporary full time position will be available until March 31st, 2024 and will be based at our
**Central City Tower** offices located in
**Surrey, BC,** with access to rapid transit, shopping, restaurants, on-site gym and other amenities.

**Your responsibilities will include**:

- Functioning as a strategic resource for site clinical teams and Health Care Support Workers
- Ensuring program objectives are met within Fraser Health owned & operated Long Term Care (LTC) and affiliated/non-affiliated LTC and Assisted Living (AL) sites
- Providing expert advice related to the program's mandate
- Coordinating activities related to Health Care Support Worker onboarding at the site level
- Managing a system to track individual Health Care Support Worker practicum/in class training rotations
- Provides leadership to roles/teams within Integrated LTC and AL to support staff and students within the sites
- Developing and/or recommending tools and mechanisms to measure, monitor and evaluate progress and identify outcomes of the HCAP program
- Managing program communications and providing leadership to teams
- Developing and maintaining cooperative working relationships with clinical leaders

**Your qualifications include**:

- Bachelor's degree in Nursing, Allied Health Science or other relevant clinical discipline.
- Seven (7) years of recent related clinical experience in a long term health care environment including five (5) years of experience in managing and leading clinical initiatives/programs, or an equivalent combination of education, training and experience.

Detailed Overview:
**Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families**:
Leads the Health Career Access Program (HCAP) implementation within the areas of Fraser Health owned & operated Long Term Care and affiliated/non-affiliated Long Term Care and Assisted Living (LTC & AL) sites. Provides expert advice related to the program's mandate, integration of Health Care Support Workers into site operations, and assists sites to identify and raise student progress/performance issues as required. Develops tools and mechanisms to measure, monitor and evaluate progress and outcomes of the program within these settings and provides related advice and support to those with operational accountability. Leads program communications and dissemination of information across LTC & AL, serves as a key resource to program queries and issues and works collaboratively with a variety of stakeholders. Provides leadership to roles assigned to support the program within Integrated LTC & AL. Responsibilities:

- Leads and coordinates the Health Career Access Program (HCAP) implementation within the areas of Fraser Health owned & operated Long Term Care (LTC) and affiliated/non-affiliated LTC and Assisted Living (AL) sites and functions as a strategic resource for site clinical teams and Health Care Support Workers, ensuring program objectives are met within these settings.
