**POSITION SCOPE**:
The Circle of Care Coordinator will be responsible for the case management and direction of individualized treatment planning, service delivery, and treatment review for each client. The Circle of Care Coordinator will take the Lead in the review of clinical issues and program policies and procedures, as it relates to the Circle of Care.

**REQUIRED QUALIFICATIONS**:

- Mandatory Requirements_
- Bachelor of Arts Degree in a Social Work or Equivalent
- 5 years’ work experience in social/health settings
- CPIC (Vulnerable Sector) - must be provided at the time of employment
- Additional Requirements_
- Excellent computer and technology knowledge and skills,
- Knowledge of Electronic Medical Record technology,
- Excellent written and verbal communications,
- Ability to provide basic counselling services,
- Excellent facilitation skills,
- Efficient time management skills and organizational skills in an office setting

**KEY RESPONSIBILITIES**:

- Clinical_
- Is the first point of contact for client referrals and coordinate children’s clinical therapeutic services,
- Will provide assistance to individual(s)/families through the case management model,
- Supports evaluation of program services to enhance patient outcomes and acts as a positive change agent to implement clinical initiatives to increase quality client care,
- Supports the overall development and implementation of a comprehensive, individualized plan of treatment in collaboration with other team members and service providers as needed,
- Ensures the incorporation of the client’s wellness plan, psychosocial functioning, and strengths in the plan of care,
- Participates in the planning for therapeutic interventions as appropriate for clients in a crisis,
- Programming_
- Develop and implement programs that address the needs identified in the Community Health Plan,
- Collaborate and network with other departmental staff when developing programs and
- Assist in the development of culturally appropriate educational and promotional materials
- Administrative_
- Participate in performance evaluations as established by the Atikameksheng Anishnawbek,
- Establish educational goals in the form of an annual learning plan,
- Participates in program planning and organizational staff meetings,
- Record participation statistics (data entry) and prepare monthly reports,
- Contribute to report submissions to meet funding requirements and annual reports,
- Make recommendations for policy and manual revisions on an annual basis,
