PROGRAM: Seniors Coordinated Outreach

Seniors Coordinated Outreach offers service through outreach workers and support workers hosted at organizations throughout Edmonton. Seniors Coordinated Outreach services provide support to seniors to help them connect to the supports they need to thrive.

Over the past year organizations across Edmonton have collaborated to redesign Seniors outreach to provide a coordinated approach to accessing services and will be testing the new approach over the next two and a half years. The Support Worker role will work collaboratively with Seniors Outreach Workers and other organizations across Edmonton to help connect seniors to services in our community.

The Seniors Support Worker provides direct support to seniors by supporting them to access programs and services to improve their quality of life. The Support Worker helps with referrals to services, assists with basic navigation of programs, arranging appointments as needed, and aids with completing forms for government and community programs. If a participant requires consistent medium to long term needs for support or have more complex needs, they would be directed to a Seniors Outreach Worker for additional support.

Seniors Support Workers are asked to use an approach that places the unique experiences and identity of the Senior at the centre of service delivery. We aim to provide service in a participant-centred manner, that honours the dignity and strengths of the seniors we serve. Seniors Coordinated Outreach highly values and prioritizes active consideration of equity, cultural and social diversity and aims to embed building thoughtful relationships that honour the diversity of the participants.

**RESPONSIBILITIES**:
Intake and Screening
- Provide initial intake and screening for referrals, determine presenting issues, and surface support needs, service barriers, and cultural needs.
- Address presenting issue and support needs if able and/or triage to Outreach Worker.

Information and Referral Services
- Works to build relationships with seniors in the community and educate about services and supports available to them.
- This may occur in any variety of on-site and off-site locations where seniors frequent.
- Provide information and/or referral(s) to appropriate government and community resources to older adults and/or family members as required.
- Assist in completing forms as required by participant and/or family for government benefits such as pensions, health care coverage, accessible transportation and housing.
- Support the participant by making referrals to social programs and community engagement opportunities.
- Accompany to services where supported referrals are needed/appropriate.
- Provide telephone, home visit, and community visits with seniors to help address their needs in navigating services as appropriate.
- Maintain a working knowledge of resources available to older adults.
- Provide regular participant follow up to assess additional needs on a short-term basis.
- Refer to Outreach Worker as appropriate.

Community Partnerships and Collaborations
- Establish and maintain strong working relationships with community agencies providing outreach services and work with the program’s Practice Coordinator held at ESCC.
- Collaborate with the program’s Outreach Workers and Senior Support Workers across the city.
- Regularly attend and participate in the Community of Practice. Use this space to build relationships and strengthen collaboration.
- Actively work to build skills as a Senior Support Worker during sessions.

Consistent communication with the Practice Coordinator to learn about adjustments to the program and provide feedback on the new model.

Program Development and Evaluation
- Support Program Evaluation utilizing approved Evaluations Metrics and Tools for the program (E.g. surveys, focus groups, and observations.)
- Work with your organization to provide feedback to the Practice Coordinator on ways to improve service.
- Implement changes to service as requested by your supervisor.

Records Management
- Complete and maintain accurate participant records, including intake, assessment, and notes.
- Administering outcome measurement protocols and data entry into required systems.
- Adhere to data privacy and information sharing determined by the organization.

General Activities
- Support program volunteers as required.
- Attend internal meetings and contribute to committee work as needed.
- Ensure confidentiality is maintained as per organizational policy.
- Adhere to all organizational policies and procedures.
- Other responsibilities as required.

**QUALIFICATIONS**
- Education in gerontology, social services, human services and/or extensive experience working with older adults.
- Strong assessment, interpersonal, and decision-making skills are required.
