Lieu de travail : Welland ou Hamilton avec déplacements fréquents entre les deux régions

Sous l’autorité de la Direction des Services Spécialisés, le(la) travailleur (euse) communautaire en soins périnataux accompagne les nouveaux parents et leurs nouveau-nés dans les régions desservies par le CSCHN en fournissant des conseils personnalisés sur la nutrition et la santé globale durant les périodes prénatales et postnatales.s.
Responsabilités:
- Offrir des sessions de consultation individuelle prénatales et postnatales relativement aux soins de santé de la mère
- Travailler en collaboration et en consultation avec le personnel médical du CSCHN pour desservir les clients du programme
- Contribuer à améliorer la santé globale et le bien-être des familles ayant des bébés
- Créer et promouvoir un système de soutien pour les nouvelles mamans et leurs nouveau-nés
- Promouvoir l’allaitement maternel ainsi que sa durée en offrant un soutien prénatal et postnatal
- Planifier, organiser et offrir des activités de groupes, tel que des cours prénataux, des groupes éducatifs postnataux, des groupes de parents
- Offrir un suivi postnatal sur les soins nutritifs aux nouveau-nés
- Préparer collations et plats froids lors d’activités de groupes
- Faire des visites à domicile et des consultations individuelles
- Prévoir et préparer le matériel nécessaire pour enseigner et démontrer les éléments choisis lors d’une visite
- Encourager l’établissement et le maintien de relations solides entre les parents et les enfants
- Tenir les statistiques et les dossiers prénatals et postnatals à jour et préparer des rapports
- Participer aux réseaux/comités pertinents à la demande de la superviseure
Qualifications:
- Diplôme collégial ou universitaire en santé publique ou un autre domaine en santé et services sociaux obtenu d’une institution reconnue
- Détenir un certificat sur l’offre des soins prénataux et postnataux (ou être disposé (e) à suivre cette formation)
- Excellente connaissance du français oral et écrit, bonne connaissance de l’anglais
- Deux à trois années d’expérience en santé pulique ou dans le domaine de santé/services sociaux
- Expérience en recherche et planification, en promotion de la santé et gestion d’activités et de projets
- Excellentes capacités de communication (incluant l’aisance à animer des groupes et présenter des ateliers), de relations interpersonnelles et de résolution de problèmes
- Connaissance des problématiques reliées aux déterminants de la santé
- Engagement envers les interventions axées sur la collectivité
- Connaissance des ressources communautaires appropriées au programme
- Connaissances approfondies des besoins spéciaux des nouvelles mères
- Aptitude à travailler avec des mères enceintes, des pères, et des nouveau-nés
- Faire preuve d’initiative et être capable de travailler d’une façon autonome
- Être disposé(e) à travailler selon un horaire variable y inclue des soirées pour répondre aux besoins de la clientèle
- Posséder un permis de conduire valide de l’Ontario et avoir accès à un véhicule automobile
- Être confortable à effectuer des visites au domicile des clients
- Fournir une vérification d'antécédents judiciaires en vue d'un travail auprès de personnes vulnérables
- Une preuve de vaccination contre la COVID-19 ( ou un billet médical pour une exemption) sera demandée au moment de l’embauche
