**Reports to**: Housing Manager
**Wage Grid**: $24.50 - $26.50
**Hours/Week**: 28
**Program Description**:
The Housing Team is a combination of Housing Programs, providing outreach supports, case management and subsidies. These programs are designed to break the cycle of poverty, homelessness, and criminal behaviour by aiding participants in finding and maintaining safe and affordable housing. Using strength-based and client-driven models, we work together with participants in determining and achieving their goals.
**Summary of Position**:
The Housing Case Manger’s role is to build positive, therapeutic relationships with participants, work with landlords and other service providers, and promote participant self-determination. A Case Manager will visit participants in their homes and in the community, provide wrap around support, and engage with participants in setting and achieving their goals. The Case Manager is an integral part of the Housing Team and works collaboratively with other case managers and management staff to effectively carry out program deliverables.
Under the supervision and direction of the Housing Manager, this position participates in program development, implementation and evaluation strategies and acts as an identified contact for other professionals and participants.
**Key Duties and Responsibilities**:
1. Assessing eligibility and prioritization for housing support.
2. Engaging in participant driven goal setting and case management.
3. Maintain and assist a caseload of individuals and some families with a realistic, holistic approach.
4. Report rental supplements, individual case notes, progress plans, and incidents within HIFIS and our case management system.
5. Mediating conflicts with landlords.
6. Visiting participant’s living spaces, report concerns, changes as well as reporting maintenance issues.
7. Building therapeutic and beneficial relationships with participants.
8. Be self-directed and be able to create priorities and manage time.
9. Adhere to professional guidelines with respect to client confidentiality.
10. Attend appropriate staff training sessions.
11. Network with other related service providers.
12. Ensure program guidelines are followed.
13. Flexibly in scheduling is required.
**Qualifications**:
**1. Post-secondary training**: mental health certificate, degree in human services, criminology, social work or a related field and/or an equivalent combination of post-secondary education and experience is required.
2. Combined knowledge and experience about community services that support and assist individuals.
**Working Challenges**:
When working with a vulnerable population, there can be exposure to adverse environmental working conditions. These can include verbal and physical aggression, which require safety precautions and a calm perspective in approaching the situation. Although there are challenges involved with this role, guidelines and policies are in place to support the employee in the workplace.
