The Community and LTC Coordinator is a dedicated regional resource that will support the integration of services for stroke survivors by developing and implementing a plan for the provision of stroke best practices within Long Term Care facilities and the communities within the Region.

The role encompasses four elements: facilitator, consultant, change agent, and educator.

The incumbent will:

- Collaborate with LTC administrators and Community Agency leadership in the planning and delivery of effective strategies to address needs and enhance best practice
- Contribute to Stroke Initiatives by participating on and/or leading project teams
- Provide leadership, mentoring and outreach to be available as a resource to community agencies and long term care

The broad objectives of this position are to:

- Build on the results of provincial pilot projects and ongoing initiatives (e.g., development and dissemination of best practices, sustainable relationships, templates and tools to standardize best practices).
- Support implementation of best practices for transition management and community re-engagement.
- Focus on education to support widespread uptake of best practices.
- Use current structures and/or develop additional structures, as required, to ensure that the community and long-term care sectors are active partners in the regional and provincial stroke networks.

The Coordinator will identify and build on opportunities for system change and improvement, improve stroke care practices, consistency of care and reduce service gaps. This role must work collaboratively with health care providers and provincial organizations across the continuum of stroke care throughout Southwestern Ontario. The goal of the Southwestern Ontario Stroke Network is to optimize access to best practice stroke care across the stroke continuum and specifically across Southwestern Ontario.

The unique nature of the role and responsibilities requires a high degree of self-direction, independence of action and leadership within a framework of collaboration and teamwork and broad guidelines for implementation of the stroke strategy. Mobilization of the health care communities, capacity development and facilitation of diverse groups, creative approaches and original thought, innovative solutions to challenges in systems change, care delivery, communication of patient information, data collection and evaluation are mandatory for this role.

Regular Full-Time

Rate of Pay: $46.46 per hour - $58.07 per hour

Hours of Work: 30 Hours per week

**Qualifications**:

- Degree in a health related discipline; Masters Degree preferred
- Member of a regulated health profession under the Regulated Health Professions Act with active member status
- Minimum five (5) years recent, related experience with experience in the health care system
- Demonstrated skills as an educator; knowledge of current education methodologies
- Superior presentation skills for public and professionals, including report generation and proposal writing
- Clinical experience and knowledge of the stroke system; expertise in stroke care is an asset
- Computer literate with a demonstrated knowledge of data collection, surveys, databases, analysis and evaluation; ability to understand and interpret provincial indicators and report card results
- Demonstrated ability to manage multiple projects, facilitate systems change/improvement
- Demonstrated knowledge of the continuum of care with experience in the health care system
- Critical thinking and analytical skills
- Demonstrated coaching and mentoring skills
- Computer literate with a demonstrated knowledge of data collection, use of databases, data analysis, and evaluation
- Ability to build collaborative partnerships to achieve results
- Planning, implementation and evaluation skills
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC’s Mission, Vision and Values
- Demonstrated ability to attend work on a regular basis

Immunization Requirements:

- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
- Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
