Dr. Jay Wang is a psychiatrist who is seeking a passionate and committed Registered Clinical Counsellor (RCC) with an interest to work in a team-based model as a patient case manager to provide high quality mental health care in the community to suitable patient populations. Currently, the clinic is completely virtual, with an active plan to transition to a physical clinic in 2024. The clinic aims to provide care to functioning adult populations with a mild to moderate degree of mental health conditions such as depression and anxiety, through evidence-based psychotherapeutic and medical treatment options.

**Roles and Responsibilities**

As a patient case manager, you will:

- Provide patient case management service and clinical counselling work
- Review patient file +/- patient initial meeting in order to provide continuity of care and prevent client repeated their story prior to psychiatrist consultation and follow ups
- Complete individual patient case manager follow up sessions with patients per patient request
- Plan, design, and deliver/facilitate approved group psychotherapy - including but not exclusive to mindfulness and CBT
- Facilitate educational workshops if needed
- Follow patient treatment plan as created by supervising psychiatrist and utilize therapeutic interventions to accomplish patient goals
- Complete session notes after group and individual session
- Regular care management meetings with supervising psychiatrist for patient clinical update and treatment plan discussion with associated meeting notes
- Serve as a centralized conduit between patients and supervision psychiatrist in communication and patient requests, including but no exclusive to following examples such as patient request for forms/letters to be filled, patient clinical updates, etc
- Maintain healthy and professional interdisciplinary relationships
- Attend case management and clinical meetings
- Participate in ongoing professional development (in-service and formal)
- Abide by and review code of ethics
- Adhere to clinical and ethical standards set out by the by the BCACC and other professional associations joined
- Adopt and adjust patient care management duties as required for clinical patient care and/or as required by the supervising psychiatrist

**Expectations**
- Use only evidence-based resources approved by supervising psychiatrist
- Utilize a team-based model
- Courteous and personable but with clear professional boundaries and conducts with all patient interactions
- Ability to work in a fast-paced high volume patient care team

**Mandatory Qualifications**:

- Master’s Degree in counselling or in a related field identified by the BC Association of Clinical Counsellor (BCACC)
- Maintenance of registered with the BCACC as a Registered Clinical Counsellor (RCC) with good standing
- Maintenance of professional insurance through the BCACC

**Necessary Clinical Skills and Experiences**
- At least 2 years of working experience in the area of patient care case management and/or group/individual psychotherapy
- Demonstrated skills in patient clinical assessments, pertinent history, mental status exams, and appropriate clinical decision making
- Demonstrated skills in formal documentation and report/note writing of all relevant clinical encounters
- Demonstrated strong ability to develop, coordinate, lead/facilitate, and evaluate education/training/psychotherapeutic group sessions
- Strong interpersonal skills
- Strong communication skills, including but not exclusive to verbal and written

**Other Valued Skills and Experiences**
- Basic computer and typing skills
- Organizational skills

**Work remotely**
- Yes, prior to the development of a physical clinic; following the development of the physical clinic, there will be a mixed model, at the discretion of the supervising psychiatrist

**Job Type**

Permanent, Part-time to start (20 hours per week), with the goal of moving into full-time (40 hours per week) depending on performance
- 5 hours/day, 4 days a week to start
- Plan to move towards 8 hours/day, 5 days a week in 2024

**Salary**: $35 - $38 per hour

Language Requirements: English

Other languages: not required but an asset (particularly Mandarin)
