Coordinate the patient's care to increase the efficiency and quality of care that is delivered to the patient. The Nurse

Practitioner (NP) will support and navigate the patient's surgical journey. The NP will be the patient advocate and

provide care in a highly supportive and personal manner, and serve as a liaison to ensure that patients and families

receive resources and services as needed.

The Nurse Practitioner (NP) works within the framework of nursing as member of the inter‑professional healthcare

team. The NP role will focus primarily on clinical practice requiring advanced clinical assessment, clinical reasoning,

pharmacology, and communication skills to formulate an optimal clinical management plan.

The NP reports primarily to the Director of the Regional Surgical Services Program in collaboration with the Director

of Surgical and Ambulatory Care services, with a matrix reporting relationship to the Chief Nursing Executive, and

accountability to the Chief of Surgery and the Medical Director of Musculoskeletal Health. Care is provided within the

scope of practice of an RN (EC) and as outlined in the NP collaborative practice agreement, any care that is outside

of the NP legislated scope of practice will be authorized through the use of medical directives and direct orders.

Orthopaedic Surgeons and Anesthesiologist(s) will collaborate with the NP routinely and provide direction related to

development, implementation and amendment of patient care plans. NP practice addresses the five domains of

Advanced Practice Nursing: Clinical Practice, Collaboration, Research, Leadership, and Change Agent. The

percentage of time dedicated to each of these domains is variable, based on the program needs and will evolve with

the program.

PRIMARY RESPONSIBILITIES
- Must provide patient care or services in a manner that is consistent with TBRHSC mission and reflects the core

values of Patient & Family Centred Care.
- Must treat all information concerning patient affairs or illnesses and/or hospital business as confidential, privileged

information, which must never be discussed except in the course of duty.
- Must be accountable for ensuring the safest patient care and services and for recognizing and reporting safety

incidents or potentially unsafe conditions.

Essential Functions:

- Educates and provides information to new and existing patients and families regarding preparation for surgery and

discharge, post‑operative rehabilitation, support services, community resources, and follow‑up.
- Serves as liaison for patients and families between interprofessional team members and departments.
- Ensures and supports (where appropriate) scheduling of appointments, identifies potential gaps as well as the

coordination of care for patients returning to their communities for treatment and follow‑up.
- Identifies opportunity for quality improvement, research and best practice.
- Responsible for patient satisfaction.

Clinical Practice:
The NP is a self‑directed practitioner who directs, provides and evaluates holistic patient care based on advanced

nursing and medical management knowledge and skills for a select patient population.
- Performs and documents comprehensive assessments (including documentation of medication review, side effects

of treatment, adherence to treatment plan and protocols, etc.) for perioperative and post‑surgical patients.
- Identifies and analyzes patient's physical and psychosocial health care problems
- Identifies health education needs of patients and families
- Communicates accurate diagnoses and develops a plan of care in collaboration with patients and families and the

interdisciplinary team including a review of related implications of treatment
- Prescribes pharmacological and other therapeutic interventions as per NP Scope of Practice and in accordance

with medical directives and guidelines of practice
- Orders and interprets appropriate diagnostic and laboratory tests necessary to guide the plan of care within scope

of practice and in accordance with medical directives
- Performs invasive/non‑invasive procedures related to patient population as defined through scope of practice

Modifies treatment plan based clinical finding, patient/family responses and outcomes and in collaboration with the

Surgeons
- Accurately documents assessment, plan, interventions, and evaluation of patient/family responses in the health

record and program‑specific communication tools
- Promotes wellness through education, support, and facilitation of Self Management principles for patients and

families
- Directs referrals to appropriate health care professionals or services
- Participates and/or leads the development of evidence‑based protocols
- Supports quality and risk management strategies
