PROGRAM OVERVIEW

Vancouver Aboriginal Health Society (VAHS) is a not-for-profit organization located in the Downtown

The VAHS Women's Mobile Primary Care (WMPC) program addresses the pressing health care needs

- Oversee intake, assessment, and referral processes to ensure a seamless continuum of care
- Conduct periodic reviews of client files and case notes
- Support the Program Manager in building a case management framework for the program

providers to enhance program reach and impact
