The Regional Cardiac Care Coordinator (RCCC) position is held by is a Registered Nurse who has a strong knowledge base and nursing expertise in complex cardiovascular diseases and treatment modalities and is independently accountable for the management, education and coordination of complex cardiac care.

The RCCC partners with the interventional cardiologists, heart failure team, cardiothoracic surgical team, electrophysiologists, advanced practice practitioners, and other clinical staff including internal and external departments, physicians and Nurse Practitioners at local and regional healthcare facilities to ensure that procedures, surgeries and treatments are completed in a safe and timely manner for both in-patients and out-patients.

RCCC’s are also responsible for the quality and cleansing of data input into the CorHealth Ontario data base WTIS (wait times information system); CorHealth uses our data to improve cardiac care across the province.

**PRIMARY RESPONSIBILITIES & DUTIES**:
**Cardiac Triage**:

- Receiving/reading referrals, collecting pertinent patient information, calculating urgency, analyzing clinical data for completeness and accuracy, ensuring the appropriate care is requested by using critical thinking and effective problem-solving skills for planning and implementing evidenced based safe cardiac care.
- Triage is based on the regional level for all patients from the south-east LHIN and Ontario north.
- RCCCs ensure appropriate resources are available to support patient care and procedures.
- RCCCs conduct and document patient consultation to problem solve with the multidisciplinary team and ensuring quality follow up medical care and interventions provided.
- Participate in organizing/planning new surgeries/procedures and programs.
- Assist in the development, implementation and/or revision of program goal and objectives.
- Provide knowledge expertise, coordination, clinical guidance based on protocols to staff to ensure that we reach our goal and vision. Proactively review patient schedules to ensure efficiency in clinical workflows, identify and resolve potential conflict.

**Communication**:

- RCCC’s are the first and often sole communication and information provider to: patients, families and referring hospitals in our region. Including detailed information for pre/post procedural and operative instructions.
- We provide Health care advice to patients, families, other health care professional’s within our region.
- RCC’s provide medication instructions for before during and after cardiac procedures. We educate patient and family about signs and symptoms of coronary artery disease that require visit to ER, family physician or cardiologist. We collaborate with medical faculty, nursing personnel and other health care agencies to facilitate patient and family needs.
- The RCC advises/troubleshoots with staff, providing answers to questions, referring to appropriate resources.
- The RCC communicates with local and regional referring physician providers and clinical staff to ensure continuity of care for both inpatients and outpatients. The RCC communicates and problem solves with multidisciplinary teams to solve complex care issues in a timely manner.

**Referral/Waitlist Management**:

- The RCC is responsible for following and reassessing patients frequently on the waitlist. This includes calling and reassessing patient’s symptoms being available for pt to call us for symptom management and waitlist updates.
- Waitlist management also includes tracking and updating test results, identifying high risk test results or abnormal results and ensuring the patients are receiving appropriate care as needed.
- Collaboration with hospital physicians and leadership as to waitlist management and resource sharing.

**Information Resource**:

- Providing education to and referral process information to our own staff and our community partners: family MD’s, hospitalists, nursing staff, urgent care clinics. We also act as a resource to other coordinators, data clerks, admin staff and the GFT secretaries.
- Creating and updating patient information booklets; patient pre-procedural information booklet, homecare guide, PFO instruction sheet, pre/post-surgical care documents, pre-surgical screening documentation, fax cover sheets. Printing and distributing education materials and the CorHealth referral forms.
- Serve as a resource person to staff/physicians with Kingston Health Sciences, regional referring providers and clinical staff, the cardiovascular and electrophysiology laboratories, operating suites and inpatient/outpatient units.

**Data Management**:

- Maintaining accurate and detailed records pertaining to cardiac procedures, updating WTIS and complying with CorHealth Ontario data collections requirements.
