**About the Role**:Toronto Youth Equity Strategy Community Support Worker (CSW)
This position provides case management planning and support for emerging adults in the TYES program at Fernie Youth Services. This work includes collaborating with key stakeholders, like Probation and Parole Officers, Employers, Education Partners, and others, to ensure each client has a tailored plan, to secure wraparound supports and achieve program goals and objectives, in a client-centered, supportive and safe environment. These young adults are on Probation and Parole with firearm convictions and gang involvement.
Working collaboratively with Adult Probation and Parole Officers this position includes case management, wraparound supports and development of re-entry strategies to supported young adults throughout the City of Toronto, through direct service, coordination, advocacy, consultation and mentorship.
The TYES CSW will function as Case Manager and will have a caseload of young adults who come into contact with the justice system through probation and parole’s Guns and Gang division. The TYES CSW will function as the key professional of the participating young adult’s support team. The TYES CSW will develop individualized service plans to support community reintegration needs. The process is supported through screening, assessment, services and referrals. The CSW manages a caseload of young persons and addresses their needs by engaging with them to develop meaningful, goal-driven, tailored service plans, to meet each young person’s unique needs and strengths, with the goal of successful reintegration into the community. The process is supported through screening, assessment, services, referrals and program delivery.
**About You**:Knowledge, Skills and Experience
- Demonstrated experience with case management, mentorship and counselling with at-risk youth and/or young adults
- Demonstrated experience with and sensitivity to the client population’s cultural and socio-economic characteristics
- Demonstrated ability to work independently, while engaging with partners, colleagues and supervisors as needed, to maximize Case Management effectiveness.
- College diploma or University degree in social work, criminology or other related field
- Minimum of 1-3 years’ experience working directly with young persons in conflict with the law.
- Excellent administrative, organizational, communication and leadership skills
- Proficient knowledge and use of available computer technology and systems
- Knowledge of relevant legislation and guidelines, including but not limited to: Youth Criminal Justice Act, Child & Family Services Act, Ministry of Correctional Services Act, Education Act, Ontario including section 23 programs
- Valid Driver’s License, clean driving record and access to a personal vehicle (with mileage reimbursement)
- Police Record Check with vulnerable sector screening
- Able to work 35 hours a week with flexibility to work evenings or weekends as required
**About Salary and Benefits**:
- $49,000 to $51,000 annually
