**Job Overview**:
As a Community Rehabilitation Worker in our Supported Independent Living Program, you'll play a vital role in providing individualized services to PDD Supported Adults (Persons with Developmental Disabilities) to foster growth, build skills and ensure every individual thrives.

**Responsibilities**:
- Engage and support autistic adults 1:1 in their homes and in community settings, maintaining regular communication with the coordinator on day-to-day activities and logistics.
- Execute individualized programming under supervisor guidance to align with participant goals and objectives.
- Serve as a key worker for assigned participants providing ongoing communication with the supervisor regarding individual progress and outcomes.
- Gather and maintain accurate participant data, including daily notes and documentation, to ensure up-to-date records.
- Motivate, coach and lead participants toward Individual Service Plan and SMART goal attainment.
- Develop and prepare instructional materials as necessary to enhance participant learning and skill development.
- Collaborate with the supervisor to develop personalized teaching plans and or learning modules for assigned participants.
- Ensure OHS standards in individual’s homes or community settings are compliant with Centre policy and regulation.
- Attend all program meetings as required by the supervisor to ensure program effectiveness.
- Contribute to the ongoing development of the Supported Independent Living program.
- Other duties as assigned.
**Qualifications**:
- Diploma or bachelor’s degree in a related field or equivalent combination of education and experience.
- Genuine interest and passion to work with autistic adults or adults with other disabilities and their support systems.
- 1 year of experience working with adults with developmental disabilities.
- Familiarity with and ability to implement adult learning principles.
- Strong interpersonal communication skills, both written and verbal.
- Genuine interest and passion to work with autistic adults or adults with other disabilities and their support systems.
- Able to work both independently and as part of a team.
- Willingness to engage in continuous professional development and training.
- Reliability and strong work ethic, with the ability to consistently meet expectations.
- Adaptability to changing circumstances and environments.
- Commitment to maintaining strict confidentiality.
- Must have access to a reliable vehicle.
- Clear Police Information check, Clean Intervention Record check and valid First Aid with CPR.
**Hours of work**:
- 37.5 hours per week
- 7.5-hour workdays
- Monday to Friday, 8:30 am -5:00 pm.
- A flexible schedule may be required, with the potential for evening/weekend work.
**Location**:
- Community support workers will work in varying environments including in participant homes as well as out in the community.
- This position requires travel within Edmonton and surrounding areas.
- A vehicle is required.
