Intensive Case Manager

**About the Role**:
The Permanent Supportive Housing (PSH) Program provides a long-term, affordable housing solution with flexible and voluntary support services offered to support high-acuity individuals who are homeless with housing retention and independent living. The Intensive Case Manager will work with individuals living in the building who are dealing with significant addiction and mental health issues, (as well as other complexities such as dual diagnosis, ABI, illness etc.) to enhance their quality of life through the provision of support coordination, advocacy, and counselling.
**Role Type: New Full time, Permanent**
**Hours of Work**:
- Days (9am-7pm)
- Hours are scheduled on a four (4) week rotational basis which includes working every second weekend, and occasional holidays (Statutory holidays)
- Participation in an on call rotation is a requirement of this role, in addition to the regular hours of work

**Qualifications**:
- Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services
- Minimum one year of progressive experience in addictions or mental health; experience in concurrent disorders or trauma informed practice an asset
- Experience in social service settings with working knowledge of case management systems and planning techniques
- Food Handlers Certification an asset
- Previous experience in housing preferred
- Lived experience related to addictions and/or mental health an asset
- CSC Clearance conducted by PWGS Canada
- Standard First Aid/CPR, Level C
- Valid G Driver’s License (G2 or G) and Satisfactory Motor Vehicle Record
- Reliable vehicle and willing to transport program participants
- Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) acknowledgement of scope of vehicle use by Insurance provide
- Mandatory on call duty, on a rotational basis
- Must be able to attend work (travel to PSH building) within an hour of notification, when on call
**Role Overview**:
Direct Case Management:
- Intake and Assessment
- Process & accept referrals from community partners
- Obtain all appropriate consents & authorizations for tenants’ participation in the program and to ensure effective communication with other service providers providing linkages, resources, referrals, etc.
- Conduct intake and enrollment activities, to include assisting tenants with gathering eligibility documents.
- Refer and link ineligible referrals to appropriate services and linkages;
- Process program discharges
Housing Assistance and Coordination of Care
- Assist tenants with move-in and support access to entitlements (income, food support, lease coordination)
- Conduct comprehensive psychosocial re-assessments and update case management services plan a minimum of every six months to include progress toward goals, modifications and/or target dates
- Create and support implementation of individualized care plans including access to health care, entitlements, life skills, community integration, eviction prevention, and treatment as appropriate in collaboration with community partners
- Create and implement crisis response plans including educating clients on appropriate use of crisis intervention services
- Support tenants with access to primary health, and other health and social services provided internally or by community partners.
- Maintain appropriate confidentiality and documentation standards as required by the agency
- Work effectively and in collaboration with the broader Permanent Supportive Housing network of providers in Guelph and Wellington
- Participate in program development activities as required
- Facilitate Crisis Intervention and supportive counseling/containment
- On a rotational basis, provide on-call services during evenings, overnights and weekends
Residential Responsibilities:
- Provide mental health and crisis support
- Participate in supervision
- Provide peer mentorship and coaching to the team
- Conduct/inform staff briefing in between shifts
- Transport tenants to appointments and other needs
- Provide light housekeeping
- Participate in program and team meetings
- Conduct unplanned tenant discharges
- Identify and mitigate tenant risks
- Collaborate with external partners
- Attend all mandatory training programs
- Rotate attendance at the weekly staff meeting
Administrative Responsibilities:
- Complete tenant documentation and other required documentation (example: program summary documents, Incident Reports, etc.)
