New employees must provide documentation/proof of full COVID-19 vaccination status, a 2-step TB screening, as well as proof of immunity to measles, mumps, rubella, and varicella (chickenpox) prior to their start date at St. Mary’s General Hospital. Staff will require clearance from Employee Health and Wellness before they are able to begin any position within SMGH.

**Position Summary**:
The Regional Cardiac Care Data Clerk (RCC Data Clerk) is a supportive role to the Regional Cardiac Care Coordinators (RCCC) in each procedure program within the Regional Cardiac program at St. Mary’s General Hospital. The RCC Data Clerk supports CorHealth/waitlist management and referral management. Is responsible for the input, data integrity of information into the CorHealth Data base and SMGH HIS for cardiac procedures.

This role plays an integral part in communicating with the RCCCs, referring physicians and hospitals, patients/families and CorHealth. The RCC Data Clerk provides clerical support functions to the RCCC in order to maintain efficient patient flow and communication including booking and registering patient procedures and tests.

**Position Requirements**:

- Successful completion of Grade 12 or equivalent.
- Completion of a Medical Terminology Course.
- Successfully able to keyboard at a speed of 50 w.p.m with mínimal error and accuracy.
- Effectively able to work with mínimal supervision.
- Able to sensitively respond to inquiries from staff, volunteers, patients, family members and members of the general public in a timely manner.
- Successfully able to perform their function in accordance with prescribed standards that focuses on the priority of People Safety and the delivery of the highest standard of Quality Care.
- Demonstrate the physical and mental capabilities necessary for proper and efficient performance of the essential duties of the position.

**Position Responsibilities**:

- Support, and work in conjunction with, the Regional Cardiac Care Coordinators (RCCC) for Cath Lab, to ensure timely and appropriate access to care for patients referred for the Cath Lab
- Ensure all referrals are entered into the CorHealth database, and removed from the CorHealth database, in an accurate and timely manner
- Review referrals upon receipt to ensure necessary information has been included
- Become familiar with the CorHealth Data Definition Dictionary and use standard CorHealth data definitions for data entry
- Comply with CorHealth and hospital privacy policies to ensure confidentiality of data
- Update CorHealth database routinely with changes in patient health status, cancellations and delays, physician/patient decisions that affect readiness to treat, and new patient information as it becomes available
- Ensure the process of data collection, data verification, data entry, and data reporting is timely and accurate
- Produce, review and distribute CorHealth reports monthly and as required
- Collects data around operational and procedures completed for SMGH monthly operational stat reporting
- Routinely audit CorHealth database for accuracy of data and missing data
- Attend monthly CorHealth webinars
- Attend CorHealth training as required
- Enter the various required data indicators into CorHealth data base for each cardiac procedure for the cath lab
- Enter the procedural report into the Cardiac procedural Report Database for each procedural case completed
- Maintains ongoing communication with the RCCC
- Maintains ongoing communication with CorHealth provincial office
- Communicate with the RCCCs, other data clerks, physicians, nurses, health records staff, patients/families to ensure the collection of high quality, accurate and complete data
- Answer phones, respond to patient/hospital/physician office inquiries, document clear messages, and refer questions to RCCC when appropriate
- Provide CorHealth information to newly referred patients by mailing out letters
- Maintains confidentiality
- Maintains good working relationships with community partners
