Concurrent Disorders Case Manager

**Scope**:
This position is under the direct supervision of the Mental Health and Addictions Program Manager and overall supervision of the Executive Director. The Concurrent Disorders Case Manager will develop holistic, culturally-based supports to recovery and wellness.

**Responsibilities**:

- Providing respectful, culturally relevant services to community members consistent with the mission, vision, and values of the organization.
- Use holistic approaches with culturally-based tools to help identify spiritual, emotional, cognitive, and physical strengths and healing paths.
- Identify goals and objectives in conjunction with clients and identifying appropriate internal and external resources to meet individual and family needs.
- Working with clients and family members to gather the essential stories needed to develop support plans.
- Network with external agencies to provide comprehensive services.
- Accompanying community members to appointments as needed.
- Providing informal individual support and group-based activities.
- Provide support and ensure the quality of services through the monitoring of all service providers.
- Keep accurate case notes in EMHware, and ensure all notes are inputted in a timely manner.
- Provide statistical information as required.
- Other duties as Required.

**Qualifications**:

- Must have post-secondary education in a field such as Addictions, Counselling, Social Work or experience in related field.
- Must have knowledge of Indigenous culture.
- Experience in case management, counselling, crisis intervention and file maintenance.
- Have or willing to obtain CPR/First Aid and other required trainings.
- Must be willing to provide a clear Vulnerable Sector Check.
- Must have a valid driver’s license and reliable transportation.
- Must be a role model in the community and the workplace.

**Salary**: $52,000.00-$56,000.00 per year
