**Role Overview**

We are seeking a dynamic and innovative individual who will work enthusiastically to deliver recovery oriented, client and family centered and comprehensive services that meet client needs, ensure client safety and work within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services. Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful individual will embed this guiding principle into their work.

**About the Team**

The **Mobile Crisis Intervention Teams (“MCITs”)** are collaborative partnerships between participating Toronto area hospitals and the Toronto Police Service (“TPS”). MCIT pairs a mental health nurse and a specially trained police officer to respond to situations involving individuals experiencing a mental health crisis.

In many instances, after MCIT provides their immediate intervention, individuals may consent to receiving short-term follow-up supports to assist in personal recovery and, where needed, referrals to longer-term community supports. This second stage of service is MCIT Follow-Up support.

We are presently seeking to recruit an MCIT Follow-Up Case Manager who will deliver recovery oriented follow-up supports as part of an integrated and comprehensive MCIT service. This individual must have significant clinical experience working with people experiencing situational crises and are living with mental health and substance use challenges.

He/she/they must also possess extensive knowledge of community-based services in Toronto, and how to coordinate access to them for the people they support.

**Role Description**

The **MCIT Follow-Up Case Manager** will receive referrals for individuals identified through the initial MCIT intervention who have consented to receiving further short-term follow-up support in the community. MCIT Follow-Up Case Manager provides short-term supports to assess immediate risk factors, assists individuals to create personal safety plans, establish priority goals, and, where required/desired, facilitates referrals to other community-based services for longer-term support.

**Summary of Duties**:
Responsibilities of this position include but are not limited to those below:

- Engage with individuals to assess needs and determine existing connections to community-based mental health & addictions services
- Re-establish/mobilize existing connections to community-based mental health services (e.g., Mental Health Case Management, ACTT, primary care )
- Provide short-term support (up to 3 months) to meet the individual’s immediate/priority needs and assist in service navigation and connections to longer-term support options
- Maintain ongoing communication with MCIT when needed and appropriate.
- Interact directly with referral source, physicians, and other services involved in client’s care
- Initiate services to clients at the point of intake to ensure that proper assessment and initial service response occurs
- Develop and lead coordinated care plans, safety plan with/ for clients and complete the Ontario Common Assessment of Need (OCAN) and other assessments as required
- Facilitate for clients’ discharge when appropriate by developing a comprehensive discharge plan
- Collaborate with other systems, organizations and individuals from courts, probation, safe beds, hospital and community to ensure a well-coordinated service system
- Record client status/progress using the appropriate documentation system

**Summary of Qualifications**:

- A combination of a post-secondary degree/diploma in a related field and 4 years’ direct experience, assessed as being relevant to the specific target population being served OR A combination of BSW, MSW or other regulated health care professional designation
