Position:
Reporting to the Administrator and working in conjunction with the Director of Care, the Resident Relations Coordinator (RRC) is a member of the Leadership Team and demonstrates a commitment to the quality of resident care by providing services in accordance with Shalom Village’s vision, mission and values. The Resident Relations Coordinator is responsible for coordinating and overseeing the social services aspects of resident care and the administration process.

KEY RESPONSIBILITIES (but not limited to):

- Plan, develop, implement, and evaluate individual social work services and/or support groups to meet the identified needs of residents and families.
- Leads the new admission process including but not limited to meeting with new residents and families/Powers of Attorneys on the date of new admission. Support the resident and family to provide consents for care and services, as well as introducing the resident/family/POA to Shalom Village and its operations.
- Lead the interdisciplinary care conference process for each resident on admission, annually and more often as needed.
- Support Shalom Village’s Family Council by being the Assistant to the Council and related activities.
- Collaborate with the interdisciplinary team and be the home’s main liaison with Ontario’s Home and Community Care Support Services to ensure bed offers are followed up on, and new admissions are planned and beds are filled in a timely manner.
- Lead the Essential Family Caregiver Program and its ongoing development.
- Assist with the provision of a resident advocacy program through the promotion and adherence of the Resident’s Bill of Rights and support development and implementation of satisfaction surveys.
- Collaborate with the Nursing Leadership team to support the work of our Finding Your Way Team (formerly BSO).
- Collaborate with the Nursing Leadership team to support Palliative Care and related research.
- Act as a resource and ensure effective communication to residents and families about the operation of the Home, as well as long-term care standards and requirements of the Fixing Long Term Care Act.
- Assist and support residents and families with adjustment to living in a Long-Term Care Home.
- Provide effective professional psychosocial and social work services to meet resident needs as identified by using psychosocial assessments.
- Assist residents and families in administering and accessing power of attorney, public guardian trustee, and other financial resources available.
- Performs other duties as assigned.

Qualifications:

- Degree / diploma in Social Work or Social Services Work from a recognized educational institution.
- Current Membership in the Ontario College of Social Workers and Social Service Workers or eligible for membership with the Ontario Association of Social Workers (OASW), the Ontario College of Social Work.
- Experience in a long-term care setting with specialized knowledge of Gerontology, Responsive Behavior Management and Palliative Care.
- Experience in techniques of social work assessment and a variety of treatment modalities.
- Knowledge and experience with all relevant Accreditation Standards, Legislation and Regulations governing Long Term Care including the Fixing Long Term Care Homes Act and Ontario Regulation 246/22.
- Applicants must also provide proof of a negative result from a 2-Step TB Skin test (dated within 6 months) before employment can begin.

**Salary**: $60,000.00-$65,000.00 per year
