Position Title: Mental Health Counsellor

Status: Part-time (37.5 hours biweekly)

Reports To: Executive Director

**Position Summary**

The Mental health counsellor will function as a member of an interdisciplinary team providing direct client support through assessments, individualized supportive counselling, and collaboration with the interdisciplinary team as required.

The Mental Health Counsellor will actively participate in the development and implementation of health promotion programs with individuals, families, and community groups.

**Duties and Responsibilities**

**Patient Focused**
- Develop therapeutic relationships with children and adults to provide individualized mental health assessments and supportive counselling
- Promote mental health, wellbeing, and resiliency through skill-building related to self awareness, emotional regulation, coping mechanisms, and interpersonal skills
- Support clients to make and sustain positive behavioural changes
- Facilitate and provide guidance for clients to access appropriate community resources
- Planning supportive group and/or community programming aimed to enhance life skills and mental wellbeing
- Occasionally attend community events to represent PCCFHT and promote our programs to community members

**Team Focus**
- Document client notes in a clear and concise manner within the Electronic Medical Record
- Collaborate with other interdisciplinary healthcare professionals as needed to provide team-based care
- Communicate with community agencies on client care issues
- Ability to work both day and evening hours
- Effectively utilize human and material resources
- Other duties as assigned by Executive Director

- Commitment to anti-oppressive practices
- Adhere to PrimaCare Community Family Health Team (PCCFHT) policies and procedures
- Participate in quality improvement and program evaluation projects
- Act in accordance with Ontario’s Personal Health Information Privacy Act to protect clients’ privacy and confidentiality

**Education, Skills and Experience**

**Education**:

- College diploma or Bachelor’s degree with a mental health-related focus
- Suicide Intervention training (ASSIST) and Non-Violent Crisis Intervention training an asset

**Experience**:

- 2-3 years of community mental health care experience preferred

**Additional Skills/Abilities**:

- Ability to work independently for extended periods of time
- Commitment to client-centered care
- Demonstrated ability to work in a collaborative practice model with multidisciplinary teams
- Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills and advanced decision-making, problem recognition and problem solving skills
- Experience with program development, quality improvement initiatives, and program evaluation
- Effective organizational, interpersonal and communication skills, and demonstrated negotiation and conflict resolutions skills
- Excellent written and verbal communication skills
- Knowledge of Microsoft Office and Electronic Medical Records
- Recognition of the limitations of own competencies and when to consult or refer to another healthcare provider

**Demands and Working Conditions**

**Physical/Mental Demands Include**:

- Minimal lifting, pulling, pushing, stretching or physical exertion
- Walking, standing, sitting
- Ability to sit for occasional extended periods of time
- COVID-19 Vaccination required
- Police records check including Vulnerable Sector Screen to the satisfaction of PCCFHT
