Autism Nova Scotia delivers programming in Halifax and in nine communities across Nova Scotia through our Regional Autism Centres. Our three program divisions are: Social & Community Inclusion; Employment Support; and Family and Caregiver Support.

**POSITION TITLE: PERSON DIRECTED PROGRAM FACILITATOR (HALIFAX)**

**POSITION DETAILS**: Full-time (40 hours per week, 8:30-4:30 Monday-Friday, evening flexibility required)
**POSITION LOCATION**: Halifax, NS
**POSITION DATES**:Start date ASAP until March 2025

**DIRECT REPORT**:Person Directed Planning Program Coordinator

**What you’ll be doing;**

At Autism Nova Scotia, **The Person Directed Program Facilitator **will be facilitating two social inclusion programs that are designed to support people with disabilities, ages 16+ (including autism, intellectual disabilities, and mental illness) in building a life of fuller participation and self-direction within their communities.

The program is informed by a Person Directed facilitation approach that revolves around planning with and walking with individual participants as they journey through skills development, supports, and connecting broader to community-based opportunities and circles of support based on their individual goals.

**Responsibilities include**:

- Facilitating Autism Nova Scotia’s ‘Person Directed Planning Program’
- Supporting program coordinators in curriculum, training, and resource development and revisions
- Collect data, conduct intakes and assessments as part of ongoing program evaluation and compose regular reports on participants’ progress in programs.
- Assist people with disabilities (including autism, intellectual disabilities, and mental illness) in moving through a PATH and other person directed planning tools to support value and goal exploration and identification.
- Facilitate and support individuals in implementing their person directed plan by identifying and accessing programs, supports and services to increase community participation, skill development, and connections.
- Communicate with participants and, where appropriate, connect individuals to support on an ongoing basis.
- Attend bi-weekly rounds meetings with program teams and coordinators
- Conduct community outreach and participant recruitment
- Responding to participant enquiries in a timely way.
- Facilitate participant intakes and onboarding of participants
- Adhere to all policies and procedures including those identified in the Policies and Procedures Manual and the Staff Manual
- Other duties as required

**Qualifications**:

- A post-secondary education in a related field/program (preferability psychology, recreation, social services or related field) preferred or equivalent experience accepted
- A valid, clear criminal record check and child abuse registry search is required
- Non-Violent Crisis Intervention, ASIST training and additional certifications preferred

**Minimum Knowledge and Experience**:

- 1-3 years’ experience working with individuals with autism or developmental disabilities is considered an asset.
- Experience in working with program development and facilitation an asset.
- Training and experience in methods of person directed planning and facilitation (such as PATH, MAPS and Circles of Support) considered an asset

**Minimum Skills and Abilities**:

- Strong working knowledge of evidence-based strategies for supporting individuals with autism, developmental disabilities or mental health challenges.
- Strong person-directed, trauma informed facilitation skills
- Ability to form professional relationships with community partners, participants, families, and colleagues
- A high level of professionalism and judgement regarding participant interactions, relationships, and safety
- Excellent communication skills (written and verbal)
- Calm professional demeanor under pressure
- Self-starter who can work independently and enthusiastically with our team
- Strong problem solving, critical thinking and ability to practically and professionally adapt
- Proficiency or experience in internal communication software (such as MS Teams) and external communication including excel spreadsheets, survey tools, Canva, and other registration programs

**Essential and Other Related Job Duties**:

- A valid Driver’s License and access to a reliable vehicle.
