**Position Title**:Integrated Hep C Outreach worker and addiction case manager

**Scope of the Position**: The primary role of the Hepatitis C Outreach Worker will be to provide intensive case management services for individuals who are infected with or at risk of being infected with Hepatitis C; as such, the Outreach Worker is to develop and to facilitate relationships and one-to-one interactions with street involved individuals. The Outreach Worker will provide social service interventions and consultation services and will act as a professional resource to a diverse, interdisciplinary team.

**RESPONSIBILITIES AND RELATED TASKS**:

- Case Identification_
- Identify individuals in the community who are at risk and link them to appropriate community resources, where indicated.
- Identify individuals in the community who are Hepatitis C positive and who may benefit from the services of the HepC Team
- Assessment Functions_
- Assess client’s current psycho-social function status, strengths, and needs and develop a treatment plan with the client.
- Life Skills Development Functions_
- Help clients set and achieve meaningful goals in the areas of medication adherence, education, life skills, substance use, relationships, productivity and leisure.
- Develop, implement and facilitate life skills interventions at the individual and/or group level.
- Peer and Volunteer Supervision_
- Provide supervision of individuals who are participating as peer and volunteers in outreach programming.
- Community Service Provider Involvement_
- Develop working relationships with community and institutional service providers that are utilized by the client population.
- Community Education_
- Provide education to service providers in the community about outreach activity programming and the HCV Team services.
- Documentation and Record-Keeping_
- Complete documentation and record keeping functions related to all client activities that meet departmental, provincial and professional standards of practice and documentation policies.
- Complete data entry functions related to client management databases in accordance with Ministry of Health requirements including Catalyst.
- Case Management_
- Liaise with the client’s family and other members of the treatment team to develop an effective support system for the client.
- Provide case management services to ensure that clients are able to access the full continuum service paradigm.
- Refer clients to appropriate community agencies and other resources as required.

**Qualifications**
- A diploma or degree in social service work or in a human service-related discipline
- Two years’ experience in working in a community setting
- Knowledge and experience of HCV
- Strong knowledge of issues affecting marginalized communities and an understanding of diversity
- Demonstrated ability to work comfortably with street-involved persons, as evidenced by experience in street-level programming.
- Experience working with diverse agencies and working successfully in partnership with these agencies
- Thorough knowledge and understanding of the local health and social service sector, government programs and current legislation that may affect clients
- Demonstrated ability to work under pressure
- Excellent interpersonal, presentation, and communication skills
- Demonstrated ability to work independently and as a team member within an evolving role in a multi-disciplinary environment
- Excellent advocacy and negotiation skills

Pay: From $23.00 per hour
